Specialist Pharmacist - Frailty

Job overview
We are looking for an enthusiastic, patient centred pharmacist to join our specialist frailty clinical pharmacy team at Harrogate and District NHS Foundation Trust. With the recent expansion of frailty services within the Trust, including a new Acute Frailty Admissions Unit and a new Virtual Hospital at Home Frailty Ward, this is an ideal time to come and join our team to help shape and develop our frailty pharmacy services. You will be joining a team that is well respected and well integrated into the frailty multi-disciplinary teams across the organisation.

Main duties of the job
To assist in the delivery of medicines optimisation and high quality pharmaceutical care as part of the clinical pharmacy service to the frailty wards including the Acute Frailty Unit and the Virtual Ward.
To develop and deliver training and supervision of various members of the pharmacy team and to support training of other members of the MDT in managing frail patients medications.
To actively participate in a speciality consultant led ward rounds and pharmacy led out-patient clinics.
To develop and lead on quality improvement programmes within frailty specialty.
To provide and contribute to the anticoagulant service.
This role includes the opportunities to be involved in a variety of trust-wide speciality groups including the management of Parkinson's Disease, Falls Prevention and Palliative Care Services.

Detailed job description and main responsibilities
To work closely with the frailty lead pharmacist to support the development, delivery and evaluation of the clinical pharmacy service to the frailty wards including the Acute Frailty Unit and the Virtual Ward.
To supervise and develop other members of the pharmacy staff in the provision of pharmacy services to the frailty specialty.
To provide expert pharmaceutical advice to members of the frailty MDT.
Person specification
Qualifications
Essential criteria
- GPhC Registration
- MPharm Degree (4yrs) or equivalent
- Completed at least Year 1 of Post Graduate Diploma in Clinical Pharmacy/General Pharmacy Practice and studying for completion of year 2
Desirable criteria
- Independent prescriber or working towards
- Member of Royal Pharmaceutical Society
Experience
Essential criteria
- Experienced in managing patients in a range of specialties including elderly care and medicine
- Minimum of 1yr post registration hospital experience or equivalent clinical experience.
Desirable criteria
- Previous experience in developing guidelines/drug policies
