Job overview
The Patient Pathway team provides high quality services consisting of managing outpatient waiting list performance, health records management, patient access to outpatient appointments and information governance that support HDFT’s short and long-term planning process and meet the needs of the Trust, Monitor, Department of Health and Local Health Community.
The team is responsible for:
· Maintaining Outpatient clinics on the Patient Administration System
· Managing the appointments system
· Preparing health records for outpatient clinics
· Providing reception cover for key desks across the Trust

Applicant requirements
The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
