Job overview

This is an exciting opportunity to join a new team and growing service, which aims to help young people at risk of developing psychosis who are experiencing an at-risk mental state.  The Therapeutic Interventions for Distressing unusual Experiences Service (TIDES) is a brand-new team working therapeutically with young people across Norfolk & Waveney.  We are seeking a Band 5 Mental Health Practitioner to join the existing team on a full or part time basis.  We will support your preceptorship process in your first year and explore further training in your second year to support your competence and proficiency.  The team office base is at County Hall, however you would be expected to work flexibly between bases in Norwich, Great Yarmouth, Kings Lynn, and home.

We are accepting those with core professions from: mental health nurses, occupational therapists, and social workers.  

 

 

Main duties of the job

We are a supported team currently made up of clinical psychology, systemic family therapy, trainee cognitive behavioural therapists and mental health practitioners (B6s) which are from a variety of professional backgrounds. Our team will be offering young people evidence-based interventions such as psychoeducational work, individual CBT and family therapeutic work aimed at helping our young people and their families to cope with the move on from their unusual experiences.  TIDES practitioners work with small manageable caseloads and hold an integrated approach to the care we provide.  Your work will be case holding of service users who are on non-CPA.

As we continue to grow you will be supported by a full MDT to include psychiatry, and support workers.   You’ll be joining a team with extensive experience of working in a psychosis pathway, and we are passionate about helping our growing team to develop their skills and understanding in this area through ongoing CPD . You will be joining a supportive environment, with regular clinical team meetings, reflective  practice and opportunities to work with the wider MDT, as well as working independently.

TIDES will see you supported through the preceptorship training in year one, moving onto the completion of further training in year two.   You will work supportively and collaboratively with the clinical team manager, those at band 6, TIDES Vocational worker and those at band 4.  The role will include the undertaking of assessments, formulation,  planning and delivery of direct service user care with appropriate supervision.  Ensuring continuity of practice and working within a multi-disciplinary and multi-agency environment.

Detailed job description and main responsibilities

Management

·         Monitor the practice of junior staff and support learning and development needs.

·         Maintain own and supervise staff in maintaining professional, therapeutic relationship/boundaries with patients, carers, and colleagues.

·         Ensure provision of regular line management and clinical supervision as well as appraisals are done in line with trust policy.

·         Support service audit systems.

 

Clinical

·         To be accountable for own professional practice.

·         Support the development and implementation of recovery focused interventions to patients with at-risk mental state.

·         Initiate, create, and maintain therapeutic relationships with patients and carers and relevant others, ensuring that clinical practice is recovery focused and patient cantered. 

·         Promote the involvement of service users, carers, and relevant others in the development of the care pathway.

·         Ensure that all patients are given a full explanation on their care pathway/treatment options and no decision is taken forward without their involvement and consent.

·         Provide training and support to others as appropriate, including non-statutory agencies.

·         Demonstrate effective personal planning and organisation of personal daily workload and that of others.

·         Work flexibly, managing own working time effectively according to service demands.

·         Make clinical judgements, identifying alternative courses of actions via problem solving, managing, and providing a high quality of care and supporting others to do the same.

·         Maintain respectful, effective professional interpersonal relationships with others.

·         To keep all relevant statutory and non-statutory agencies, including primary health, fully involved in the care plan process, through verbal and written communications.

·         To contribute to protecting vulnerable adults, older people, and young people by taking the necessary actions consistent with current legislation and Trust policies and procedures.

·         To highlight and raise concerns regarding risk and risk management of service users/carers and others.

·         To support care coordinator in the implementation of risk management where indicated.  Including the development of a crisis plan in consultation with the user, carer, and multi-disciplinary team.

·         To support service training to other professionals and agencies that may come into contact with people with at-risk stage presentations, supporting understanding of the signs and symptoms of illness and how to refer to the service.

Professional

·         Ensuring a working knowledge of at-risk mental state and the use of CAARMS in screening.

·         Demonstrate practice is reflective and informed by best practice including the use of NICE guidelines.

·         Adhere to local and professionally prescribed codes of conduct and practice. Maintain personal standards and ensure self and others adhere to all policies, procedures, guidelines, and standards set by the Trust and relevant bodies. To comply with all Trust Policies, Guidelines and Protocols.

·         Develop and maintain a professional portfolio which demonstrates engagement in reflective practice and meets the NMC requirements for PREP.

·         To work co-operatively in the partaking of regular performance reviews of personal practice in the context of the post. To use monthly line management and clinical supervision as per Trust Policy.

·         To be aware of one’s own limitations and to seek advice where necessary from appropriate senior practitioners/managers.

·         Identify any personal learning and training needs through supervision on a regular basis.

·         Support and develop the principles of joint partnership working and to be pro-acting in fostering positive inter-agency relationships and information sharing.

·         Be supportive of student placements and their learning outcomes; working collaboratively with the team to ensure that the clinical experience of student nurses is related to the educational curriculum.

·         Ensure yearly mentorship updates are made.

·         Attend all mandatory training as required by the Trust and to undertake further training and updating as required to carry out the role.

·         Ensure that personal appearance, practice, and manner of self and staff conform to the standards and expectations of the Trust.
