Norfolk and Suffolk NHS Foundation Trust provides child and adult mental health services, learning disability, wellbeing, older people's and eating disorder services across Norfolk and Suffolk.
At any one time, around 23,000 people in Norfolk and Suffolk are receiving care and attention from the Trust. About the Role: As a Community Mental Health Nurse, you will play a pivotal role in the planning and delivery of direct patient care. You will work closely with a supportive team to ensure continuity of practice, providing essential mental health care services to individuals in need.

The post holder will undertake the planning and delivery of direct patient care with appropriate supervision, ensuring continuity of practice and working within a multi-disciplinary and multiagency environment.
- Managing a caseload and the provision of Mental Health Care services to people with mental health care needs. Support and advising to other health care professionals. To support aspects of service development.
- Provide an appropriate learning environment for relevant University students.
- To be responsible for accurate mental health documents of all patients on admission, discharge and at review.
- Have an excellent knowledge of mental health conditions, relevant medications and treatment.
- Have experience or an understanding of working with people who have dementia.
- demonstrate ability to produce excellent highly detailed, assessments and risk management plans
This is an exciting opportunity to relocate and work 9-5 hrs between Monday to Friday.
