Patient Pathway Coordinator

The post holder will be responsible for working with site-specific multi-disciplinary teams to ensure that national data and reporting requirements are met. This will involve actively monitoring, reporting and entering data onto the Trust cancer database.
Working as a member of the Cancer Services Department, and in collaboration with others, ensure an integrated process by which the active management (tracking) of patients with cancer and the active live collection and inputting of data supports Trust compliance with National Cancer Waiting Times.
As a named link between the Cancer Services Department and site-specific multidisciplinary teams, ensure that potential breaches of the Cancer Waiting Times standard are highlighted and escalated according to Trust policy.
Provide appropriate cover support to Cancer Services working with other sites, 2 week wait referrals, histology reports, chemotherapy data sheets and radiotherapy referrals as indicated.

The Cancer Services department is looking for an enthusiastic and well organized individual to join their team in the role of a Patient Pathway Coordinator.
This is an exciting opportunity to be part of an established service that supports the Trust’s Cancer teams. We use the Somerset Cancer Register information system to collect real-time data throughout the patient’s referral, diagnostic and treatment pathways. The data aids the clinical management of the patients and informs progress against the national Cancer Waiting Times standards.
The main objectives of the role are;
- Working with site-specific multi-disciplinary teams to track patients through their cancer pathway.
- Pro-actively monitor individual cancer patients to ensure that they progress through the pathway within the standard timelines and to prevent breaches of national Cancer Waiting Times standards.
- Collect and validate clinical data for mandatory cancer datasets.
- Provide appropriate cover support across the team during periods of annual leave and absence.
We require a highly motivated team player, with excellent organizational and communication skills. You will be self-motivated and will work to a consistently high standard with the ability to manage your own time. A willingness to learn skills and the ability to work as a member of the multidisciplinary team, communicating effectively with staff at all levels will make the job an exciting and rewarding challenge.
