Job overview
The Intermediate Care therapy Team has an exciting opportunity to recruit a Community Assistant Practitioner, working out of St James Clinic serving the Intermediate Care beds at Goodwin's Hall Kings Lynn and Nordelph unit Downham Market. The role as an autonomous practitioner involves patient assessment, goal setting, implementation of therapy treatment plans with guidance from qualified OT and Physiotherapist. The team cover a wide variety of conditions within a intermediate care setting, a current UK driving licence and access to a vehicle will be essential for this role. Opportunities to be involved in Home visits and contribute to discharge planning is a key aspect of this role along with delivering rehabilitation programs.

Main duties of the job
You will need to be able to demonstrate effective communication skills, problem solving skills and be resilient and able to cope in changing and challenging situations. You will work under the direction of our Occupational Therapists and Physiotherapists, but will also be required to work autonomously. Previous experience of working within a community setting or within a therapy based team would be desirable. Patient visits in the in the community may be planned or unplanned depending on the need of the patient. Their may be a requirement on occasions to support the Urgent community response therapy team with delivering therapy at home and facilitating admission avoidance and early supported discharge strategies.

Detailed job description and main responsibilities
· Assess and treat patients with multiple conditions, sometimes complex in nature.
· Prepare for, carry out and monitor assessments and treatments in specified clinical areas, and discharge in line with predetermined department protocols.
· Modify and progress intervention / treatment using own clinical reasoning, notifying a qualified practitioner accordingly.
· Monitor patients and promptly alert a qualified practitioner when there are unexpected changes.
· Demonstrate problem solving, and contribute to the solution, working with colleagues.
· Make clinical judgements on patients’ response to prescribed treatment, making adjustments as required, and report findings to a qualified practitioner.
· Plan and prioritise own visits to patients along with the ICB Team daily handover and planning. Document visits on System One ledger.
· Ensuring the outcome of patient visits are documented in SystmOne record within 24 hours of visit.

- NVQ 3, or equivalent knowledge and experience
- Foundation degree programme, or equivalent demonstrable knowledge and skills acquired through training and experience
- Good general education
- Experience of working in a health or social care setting
