Job overview

An exciting opportunity has arisen within the Patient Safety and Quality team for a Patient Safety Incident Investigator.

This is a new and dynamic role that supports the Trust's positive patient safety culture and would be an excellent opportunity for someone wishing
to build upon existing experience and further develop their interest in Patient Safety.

Main duties of the job

The Patient Safety Incident Investigator will work Trust-wide, managing and assuring the quality and effectiveness of all types of patient safety
investigations and process as part of the Trust’s annually agreed Patient Safety Incident Response Plan (PSIRP) as required within the Patient
Safety Incident Response Framework (PSIRF).

They will work in collaboration with both internal and external patient safety stakeholders to investigate, analyse and use judgement through the collation of evidence and findings using the agreed methods within the Trust’s PSIRF. The outcome will be the identification and implementation of systems learning from a patient safety incident that will then guide sustainable risk reduction and service improvement, ensuring that a Just and Learning Culture and human factors principles are maintained.


The Patient Safety Incident Investigator will also provide senior leadership to the wider Patient Safety and Quality team.

Qualifications

Essential criteria
  • Senior manager with health related experience
  • Graduate or equivalent Health Professional qualification
  • Evidence of commitment to ongoing personal and professional development

Experience

Essential criteria
  • Demonstrable evidence of project management experience and the delivery of objectives at a senior organisational level
  • Proven experience in clinical incident investigation report writing
  • Extensive senior level experience of investigating adverse incidents, developing and implementing action plans
  • Able to demonstrate examples of positive impact/change within the organisation at an advanced level
  • Experience of developing corporate policy and strategy

Skills

Essential criteria
  • Familiarity with clinical terminology
  • IT skills with working knowledge of databases and spreadsheets analysis
  • Advanced presentation skills
  • Excellent written and verbal communication skills and ability to communicate complex and sensitive information
