Job overview

You will be required to work closely with clinicians, specialist nurses, secretaries, and all members of the Multi-Disciplinary Teams (MDT) and associated departments, to ensure that patients on a cancer pathway are proactively managed throughout their pathway in a timely manner.

You will be required to coordinate the patient journey from referral, through diagnostics, treatment planning and treatment, highlighting any potential delays in the pathway promptly.

Previous experience of working within an MDT Coordinator or Patient Pathway Coordinator role, alongside an in-depth knowledge of Cancer targets is essential for this role.

Candidates must have excellent organisational, communication, interpersonal skills and possess the ability to have difficult conversations in challenging environments.

 

Main duties of the job

The post-holder will coordinate all aspects of the patient pathway and be responsible for prioritising work, reviewing work practices, setting up, managing and ensuring effective running of Multi-Disciplinary Team (MDT) meetings. In addition the MDT Coordinator will be responsible for ensuring all cancer patients are monitored and tracked, maintaining the Somerset Cancer Register (SCR) database and ensuring information is communicated to the appropriate clinical and administrative staff within the Trust.

The post holder will highlight relevant patients for discussion at the MDT meeting with the clinical teams. The Multi-Disciplinary Team Coordinator (MDTC) will provide a comprehensive administrative service to support and cover MDTs and Patient Pathway Coordinators (PPC) to ensure the effective management of accurate and timely data to enable clinical decision-making at MDT meetings when required.

Detailed job description and main responsibilities

Be responsible for the co-ordination of the weekly MDT meetings, ensuring that dates and venues are booked and all MDT members are aware and attendance recorded at each meeting.

Be responsible for the preparation for the meeting; gathering information, identify patients, compiling and distributing the agenda to the MDT clinical team.

Organise supporting documents to ensure information is available for efficiency during meetings.

Update SCR for every patient with a confirmed diagnosis and ensure they are discussed at an MDT meeting.

Following the MDT meeting be responsible for circulating patient outcomes, recording accurate and concise information. This includes the actions from the MDT are distributed to the relevant clinicians for completion.

To be responsible for the consistency of tracking patients within any designated body sites against the national 31 and 62 day cancer waiting time targets, including highlighting potential breaches of targets to the Cancer Services Manager and the clinical team in a timely manner.

Be responsible for GPs receiving confirmation of a new cancer diagnosis within 24 hours of the patient being informed.

To challenge the progress of a patient’s journey with members of the cancer MDT, actively seek and use persuasive skills to other service providers to expedite existing appointments so patients can be seen and treated sooner.

To work in partnership with Lead Clinicians ensuring accuracy and timely completion of the clinical information recorded to meet specified deadlines for cancer waiting times (CWT) performance and Cancer Outcomes and Services Datasets (COSD) and national cancer audits.

Knowledge of all cancer sites is required to cross cover in absence of MDT coordinators for continuity of service.

To be responsible for entering patient data onto the SCR and Remote Monitoring System across body sites to support patients on Self-Management pathways.

To organise and participate in weekly meetings with Consultants, DOM’s and Nurse specialists to review and escalate patient pathways, record any actions and complete them in conjunction with cancer operational policies.

To communicate in a sensitive or appropriate way with a variety of NHS staff and groups. These include tertiary treating Trusts, in house hospital staff, community palliative care teams and GP’s to highlight issues and source information vital to maintaining the Somerset Database, and reducing delay to patient care.

To carry out any other duties as delegated by the Cancer Management team which are appropriate to the grade and to the responsibilities of the post.

Education and Qualifications

Essential criteria
  • Good general education with English and Maths to GCSE standard or equivalent
  • Evidence of IT skills and Healthcare information systems- knowledge of working with MS Office, especially Excel and iPM or equivalent

Experience and Knowledge

Essential criteria
  • Previous experience as a Multi-Disciplinary Team Coordinator or Patient Pathway Coordinator
  • Knowledge of medical, anatomical and oncological terminology
  • Experience of interpreting and extracting information form electronic systems
