Community Team Manager Older People’s Community Mental Health

 A rare and exciting opportunity has arisen to work with an established Older Persons Community Mental Health team (CMHT) which is dedicated in providing exceptional mental health care within our Waveney community. Our team works with Older People who are experiencing a range of mental health needs both functional and cognitive, therefore we are committed to delivering person-centred support that enhances the quality of life for our service users.

We are seeking a highly motivated and experienced individual to lead our Older Persons CMHT and Memory Assessment team embedded within the CMHT. As the Team Manager, you will oversee the delivery of comprehensive mental health services, ensuring high standards of care and promoting a positive, inclusive culture within the multidisciplinary team. You will play a pivotal role in coordinating care plans, managing staff, service user and carers involvement and liaising with other healthcare professionals including external agencies.

 

Main duties of the job

As team manager your key responsibilities will be to lead and manage the Older Persons CMHT, ensuring effective service delivery and high standards of care. Supervise and support a multidisciplinary team, including conducting regular performance reviews and facilitating professional development. Oversee the assessment, planning, implementation, and evaluation of care for older adults with mental health needs. Foster strong partnerships with service users, their families, and other stakeholders. Ensure compliance with relevant legislation, policies, and procedures. Monitor and manage the team’s budget and resources effectively. Contribute to the strategic development of the service, identifying opportunities for improvement and innovation.

The team works alongside colleagues delivering Working Age Adult services and enjoys a mutually supportive service delivery to service users in the Waveney area.

  • To provide daily leadership to the team and ensure the provision of high quality.
  • To ensure high quality individualised person centre standards of care are delivered to all service users.
  • Ensure the effective supervision and development of staff.
  • To lead promoting respect and advocacy for all service users.
  • To be an active participant in the multidisciplinary team, formulating care plans and utilising appropriate assessments.
  • To undertake management supervision to further develop competence and clinical practice.
  • To liaise effectively with all stakeholders in the care process, including carers and relatives, aspiring to provide an effective, seamless and integrated service to our service users.

Person specification

Qualifications

Essential criteria
  • Nursing degree or equivalent, Dip SW or Dip OT or equivalent, or HCPC registered
Desirable criteria
  • Higher degree or post graduate certificate in area of clinical / therapeutic practice
  • ILM certificate in management or equivalent

Experience

Essential criteria
  • Able to demonstrate experience at a senior clinical level including staff management/supervision experience.
  • Evidence of providing leadership in a Mental Health care environment including service development.
