Job overview

This is an exciting opportunity for 1 enthusiastic falls and bone health nurse to join the expanding team in Ysbyty Ystrad Fawr. This position is until March 2025 to meet the needs of the service 

 The role will involve autonomously examine, treat and support patients who have sustained falls or are living with the impact of falls, particularly fragility fractures

Promote evidence-based practice and act as an accountable role model

Be responsible for the provision of effective, efficient and appropriate high quality nursing care for patients who have experienced falls in the community or in the hospital setting. To manage fragility fracture (explaining diagnoses, sharing treatment options and monitoring compliance to treatment and its side effects)

The post holder will see patients within hospital and community settings and act at all times in accordance with the NMC Professional Code of Conduct and ABUHB policies. Provide responsive, flexible and patient centered services in line with NICE guidelines for the management of Falls and Bone Health.

Main duties of the job

Undertake comprehensive nursing assessments for patients with falls and osteoporosis, which may include physical examinations and comprehensive patient histories and accessing previous radiological investigations, GP letters and clinic letters.

Post holder will be supported by consultant for both face to face and virtual clinics.

Post holder will be trained as needed to deliver specialist bone health treatment in the community and expected to deliver ‘care closer to home’ and link with GP surgeries.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

  • Degree or working towards
  • Experience at Band 5 level or above

  • Community Nursing Qualification / Experience
  • Clinical Patient Assessment

Experience

  • Experience of managing own workload
  • Experience in teaching and providing professional support to nurses and other colleagues
Desirable criteria
  • Health promotion/education or rehabilitation experience
  • Experience of Palliative care setting
