Job overview

Within Aneurin Bevan University Health Board, our Clinical Futures Strategy sets out how we are moving to delivering care closer to home. Primary and Community Services are at the heart of the model and central to developing a new relationship with patients investing and strengthening community and social care services to support and treat patients in their homes and communities providing quality outcomes. In order to deliver the strategy, a secondment opportunity has become available for a District Nursing Team Manager in Tredegar.

This post is has two opportunities 

1 WTE permanent Rhymney District nursing team

1 WTE secondment 6 months Bargoed district nursing team

Main duties of the job

The post holder will ensure the provision of cost effective, efficient and 
appropriate high quality patient care. They will carry the continuing 
responsibility for the assessment, planning, implementation and evaluation of care needs and quality assurance. The community nurse team leader will line manage a nursing team ensuring the professional and clinical development of team members. They will promote evidenced based practice and act as an accountable role model. They will ensure that together with their team members they act in accordance with the NMC Professional Code of Conduct and other relevant policies.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
