Job overview

Torfaen Frailty Community Resource Team (CRT) have an exciting 37.5 hours per week permanent post available for a dynamic and motivated band 7 Physiotherapist.

Torfaen CRT consists of Rapid Response Nurses/Drs, Specialist Falls Service, Mental Health Practitioners, Pharmacy Team, Occupational Therapists, Physiotherapists, Reablement Technicians, Reablement Assistants and a Reablement Care Team.

At a pivotal time in the redevelopment of community services, the successful candidate, with the support of our integrated team leads, will lead and develop the physiotherapy service as we shape and develop therapy pathways and provision within the community sector.  Leading the physiotherapy team in the development and provision of community physiotherapy services across Torfaen locality and Ty Glas Y Dorlan Reablement facility.

Main duties of the job

We are looking for a dynamic and forward-thinking clinician, who will be committed to following best practice and the development of high-quality Physiotherapy services.


You will have extensive experience in community therapy provision, demonstrate good leadership qualities and share a passion for working in an integrated service.  

As a senior lead within the team, you will have proven leadership qualities and excellent communication skills with the ability to build networks across sites and disciplines.


The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

  • Ability to perform & cope with all aspects of manual handling
  • Ability to adapt to changes in work routine
  • Ability to independently move between sites within the health board or patients' residences as required

  • Ability to utilise clinical reasoning in the management of complex cases
  • Good communication skills - verbal, written & interaction

  • Evidence of initiative and organisational skills
  • Knowledge of relevant national and local clinical guidelines

  • Awareness of research principles and findings impact in clinical practice
  • IT literate
  • Ability to speak Welsh

Experience

Essential criteria
  • Significant Experience at band 6 level
  • Senior postgraduate clinical experience including complex case management
  • Significant experience requiring people management and staff supervision
  • Evidence of having worked unsupervised at a senior level
  • Experience in leadership, audit and facilitating learning with students
Desirable criteria
  • Evidence of leading change management
  • Experience of implementing clinical guidelines and standards
  • Experience of MDT and cross-boundary working

Qualifications & Knowledge

Essential criteria
  • Diploma/Degree in Physiotherapy
  • HCPC Registered Physiotherapist
  • Appropriate range of postgraduate clinical education
Desirable criteria
  • MCSP
  • Working towards a relevant higher qualification
  • Active participation in a relevant interest group
