Trosolwg o'r swydd

The  Frailty Team is to provide ‘hospital at home’ to people during periods of acute illness and or Reablement in order to prevent hospital admission or to facilitate early discharge.
The team works as a partner within a wider multi-disciplinary team.

The post holder will work collaboratively with and support members of the multidisciplinary team in the delivery, development and management of complex health and social care issues. In particular they will be responsible for ensuring the timely provision of a high standard of nursing assessment, advice, planning, implementation, training support and evaluation of care taking into account the social context in which the person lives. They will therefore be engaged in the planning and delivery of collaborative care packages that cross the health and social care boundaries. 

The post holder is expected to demonstrate and apply a broad range of nursing knowledge, interventions and skills. Knowledge of the principles of health promotion and preventing illness will be fundamental as will having a comprehensive understanding of the roles of others in health and social care.

 

Prif ddyletswyddau'r swydd

  • To be responsible for the assessment, planning implementation and evaluation of care for community-based patients. 
  • To work as a member of the wider primary care and community multi-disciplinary team
  • Administration of IV therapies within the community and at hospital sites where frailty services have clinics.
  • To support the Frailty Team, in developing the  Frailty Team and raising the profile of the Frailty Service within the Borough

Skills

  • Able to perform extended clinical skills; Intravenous therapy, cannulation, phlebotomy, syringe drivers, male and female catheterisation, removal of sutures
  • An understanding of the structure for the delivery for primary health care services.
  • Able to communicate succinctly and effectively verbally, in writing and electronically.
  • Able to present information effectively to individuals and groups

  • ECDL or equivalent
