Reablement is part of the wider Support @ Home team, offering an opportunity to recover from ill health and to prevent episodes of health deterioration where possible. The service offers a period of rehabilitation to enable service users to maintain, regain or re-learn daily living skills and abilities. Reablement programs are bespoke and tailored to the goals set with service users aiming to achieve their full potential whilst preventing or reducing the need to move on to longer term community services.
Main duties of the job
- Lead the delivery and development of the therapy component of the Intermediate Care and Rehabilitation Services for individuals referred to the reablement team which is based in the local authority covering the localities of Rhondda Cynon Taff.
- Be responsible for the effective treatment and management of Service users requiring rehabilitation in the community in order to maximise their independence and functional ability within the home and local environment, to prevent inappropriate admissions to and enable timely discharge from hospital.
- Provide clinical leadership and advice to team of Occupational Therapists, Physiotherapists, Therapy Technicians and support local authority support services staff to enhance service provision and clinical interventions for community patients within the locality.
The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.

Qualifications and/or Knowledge
Essential criteria
- Degree/Diploma in Occupational Therapy / Physiotherapy.
- Evidence of ability to work at Master’s Level.
- Specialist knowledge of professional assessments and interventions relevant to the rehabilitation in the community.
- Knowledge and understanding of current agenda and drivers for health and social care policy including legislation.
- Detailed knowledge and application of principles of clinical governance and risk management.
- Detailed understanding of the principles of partnership working.
- Evidence of critical appraisal/analytical thought.
- Specialist rehabilitation approaches.
- Evidence of commitment to CPD.
Desirable criteria
- Management qualifications e.g. ILM or clinical leadership.
- Knowledge of standardised and non-standardised assessments that can be used with client group.
- Interest and involvement in quality, audit and service evaluation.
- Postgraduate training in Rehabilitation.
- Detailed knowledge and understanding of professional ethics/standards and code of conduct, and their application in practice.
