Job overview

The recently developed Specialist Mental Health and Substance Misuse Housing Outreach Service are looking for a skilled and creative practitioner with knowledge of working with and supporting people who are vulnerably housed or homeless. The aim is to  deliver evidence based holistic mental health care to people within the context of an integrated multi-disciplinary, multi-agency specialist service.

The Team due to expansion now consists of staff from the Local Authority, Health, Third Sector, Trauma Informed Counsellors, Adult Social Care staff and Medics. The Team also works collaboratively with staff from the GP surgeries. This holistic approach provides essential support for adults with complex needs.

The service runs from Monday to Friday 9am to 5pm and covers CTM. Staff where possible cover a locality each however where there is sickness or a shortage of staff there will be an expectation to cover. 

The ability to speak Welsh is desirable for this post; Welsh and/or English speaker are equally welcome to apply.  

Main duties of the job

The post holder will provide support and intervention for individuals through holistic person centred assessment, treatment, intervention and evaluation of their care within a multi-agency team approach.  They will demonstrate safe, clinical decision-making and expert care and support for people within the community whether this be at home, temporary accommodation or hostel. They will take responsibility for a clinical caseload of clients referred to the Specialist Mental Health and Substance Misuse Housing Outreach Service.

The post holder will work collaboratively with the wider multi-disciplinary teams to meet the needs of people within their communities. This will generally involve working with adults who have health needs due to complications of drug, alcohol use and/or mental ill health and would support transitional arrangements for people requiring primary and secondary health support.

As this service is a newly established service the post holder will be required to contribute to the continuing promotion, improvement and development of the service through networking, and representing the service at all times. 

The post holder will be required to deputise for Band 7 nurses on occasion, this will include supervision of Band 3 HCSW's, attendance at various meetings and ensure availability to offer advice and support to housing providers accordingly.
