East Integrated Cluster Network Service Manager .
Band 8a/ Grade 15 (Health/Local Authority).
Bridgend Integrated Community Services are seeking to appoint the East Integrated Cluster Network Service Manager , to operationally manage and strategically develop the Integrated Team. The post holder is responsible to both the Local Authority and University Health Board and manages services across these agencies. The Integrated Community Network Team works to develop seamless pathways of care and support for adults with complex needs.
The role will be key in bringing in a multi-disciplinary approach to provide and implement timely professional support for people’s needs. This ensures choice and control for individuals and places the person at the centre of their care. It provides a clear expectation that the person’s voice and choice will be heard. The Teams are ambitious in looking to develop innovative and creative ways to support the individual and those important to them. It works to recognise the changing needs within communities and works collaboratively to meet them.
Main duties of the job
The Team consists of staff from District Nursing; Social Work; Occupational Therapy; Physiotherapy; Community Psychiatric Nursing; Speech and Language Therapy; Dietetic and Pharmacy and links closely with the Third Sector. This holistic approach provides a full offer of support for adults with changing needs in their localities as well as meeting complexity of needs. The manager will provide leadership and vision to the multi-disciplinary team and ensure that safe governance is met. They will enable staff to support the person they are working with and those important to them through holistic person centred practice. They will lead staff in providing an effective team around the person.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Working for our organisation
The post holder will undertake a key role in in the development of the Transformation agenda and Accelerated Cluster working in the East Cluster network area and will work closely with the Cluster Partnership and Primary Care services.
Detailed job description and main responsibilities
You will have experience of multi-disciplinary and multi-agency working preferably across traditional organisational boundaries. The Integrated Cluster Network Team’s (ICNT) function is to work in one of three defined geographical cluster networks, in the delivery of preventative, reactive and rehabilitative community health and social care services, as well as the arrangement and delivery of pre-emptive contingency care planning and managed care and support, to the residents of the cluster. The ICNT mainly focusses on adults affected by the issues of frailty, and physically disabled people who are affected by long-term, often complex and adverse conditions.
