Job overview

The post holder will work as a member of the Primary Care Nursing Team under the direction of the lead nurse.

The Health Care Support Worker will support Registered Nurses, General Practitioners and other health care professionals in the assessment, treatment and management of patients presenting in primary care settings.

The health care support worker will work as part of the nursing team within standard operating procedures, and Registered Nurses and General Practitioners are available for reference.  

 

The post holder will assist with specified delegated clinical tasks that include the provision and monitoring of Chronic Disease management providing a caring, patient focused service at all times.

The post holder will deal with queries from patients, Health Care professionals and other agencies face to face and by telephone.  The role will also involve administration and clerical duties including updating patient records on the computer system.
