Job overview

We have an exciting opportunity for an experienced and enthusiastic administrator to join our expanding Transfer of Care Admin Hub.

Working both autonomously and as part of the team, the successful candidate will work with  in close liaison with the a multi-discipline team comprising of nurse coordinators, therapists, social workers to provide comprehensive admin support to help facilitate discharges from Acute hospital Trusts.

Our team employs the latest hybrid working techniques ensuring that staff members get some opportunities to work from home but also have face to face communication with our colleagues. The successful candidate will on occasion need to be able to attend our Old Swan, Broadgreen or Aintree office sites for meetings or for training purposes.

The role provides a fantastic opportunity to creatively develop the role and work with senior team members to help shape service improvements.

Main duties of the job

To act as an administrator within the Transfer of Care Hub working in close liaison with the a multi-discipline team comprising of nurse coordinators, therapists, social workers . The post holder is also expected to update a variety of systems in accordance with multi trust patients protocols

The post holder will be required to effectively communicate with relevant agencies across the whole system including LUFHT Co-Ordinators, Intermediate Care Teams and Urgent Care Teams to collate and transfer accurate information

Detailed job description and main responsibilities

  1. To receive referrals from a variety of health and social care professionals and accurately, recording specific medical information and terminology directly onto service computer systems in accordance with service and trust The post holder is required to action any missing information in line with referral protocols and referral management service flow charts especially the TOC’s. 
  2. To respond to telephone queries from referring clinicians in acutes/CCG’s/HUBS/Primary care about their patients regarding status of referrals
  3. To participate in regular audits necessary to monitor effectiveness of service provision, and identify training issues.
  4. To be aware of status of acute and community facilities on a daily basis through RFD list and ensure all members of the team are informed of any updates.
  5. Carry out information gathering functions on a daily basis, enabling efficient provision of service and supporting the role of the Operations Service
  6. Uphold patient confidentiality at all times and adhere to Clinical Governance guidelines when dealing with patient
  7. Actively input into the development and provision of alternative pathways 
  8. Actively input into the development and distribution of service protocols ensuring members of the team observe and adhere to
  9.  To assist in maintaining links with acutes/CCG’s/HUBS/Primary care about their patients regarding status of referrals etc
  10. Effectively communicate with relevant agencies across the whole system including LUFHT Co-Ordinators, Intermediate Care Teams and Urgent Care Teams to collate and transfer accurate information, relating to service availability and pressures,

Qualifications

Essential criteria
  • Working knowledge of Microsoft Word Applications
  • Computer literate
  • Knowledge of Microsoft Office or other database software
  • Ability to use email.
  • Willingness to undertake further training/qualifications
Desirable criteria
  • RSA III (or equivalent)
  • NVQ 3 Business Admin or equivalent relevant office experience
  • Excellent Audio Typing
  • ECDL

Knowledge

Essential criteria
  • Experience of working in a busy office environment
  • Previous clerical experience.
  • Familiar with standard office practice and procedures.
  • Have an understanding of the implication of the data protection act
Desirable criteria
  • Experience of medical terminology.
