Trosolwg o'r swydd

The aim of the Haemato-oncology Pathway Navigator role is to help support and navigate patients, their families and carers through 
cancer pathways. The service’s prime aim is to be 
patient-focused and proactive in seeing that the patient 
receives the very best cancer pathway experience with 
continual communication and support. 
You will be involved throughout the patient journey from 
referral to follow up within the hospital setting, working 
directly and indirectly with patients and their carers, 
assisting the clinical team in coordinating the patient 
pathway. The role requires someone with exceptional 
organisational skills who can demonstrate great 
communication and interpersonal skills. A caring and 
sensitive attitude is essential as you will be dealing with 
patients, their families and carers.

The post holder will be expected to work independently 
within the appropriate policies and procedures for the 
service and will actively support and contribute to the 
requirements of the service and ensure compliance with 
performance targets and quality standards required at 
a local and national level.
They will coordinate care and act as a single point of 
access for patients They will work with allied NHS 
organizations to enable patients to access required 
tests as needed, This includes undertaking a Holistic 
Need Assessment at key points along the treatment 
pathway. This includes speaking to patients about what 
matters to them through this holistic conversation and 
assessment.
They will provide cancer information and signpost them 
to support services and resources and will develop 
personalized care plans which will be shared with the 
specialist teams and community services.
They will enable patients to navigate Health and Social 
care services, supporting patients to maximize their 
quality of life whilst receiving treatment at the cancer 
centre

  • NVQ4 or NVQ3 plus additional training to diploma level equivalent, or equivalent through short courses

  • Evidence of continued role development
  • Willingness to undertake learning and development courses
  • Coaching or teaching qualification
  • European Computer Driving Licence or equivalent

Experience

  • Relevant health or social care experience at Agenda for Change Band 3 or equivalent
  • Experience of multi-professional working
  • Experience in the use of data management
  • Knowledge of patient needs when being treated in a healthcare setting
  • Evidence of good communications skills
