Job overview

Our Liverpool Stroke Recovery Partnership and South Sefton Stroke Early Supported Discharge Team (ESD)  are looking for an enthusiastic team player to join our team  as an Occupational Therapist. 

Our ESD Team is an innovative and highly specialist team providing stroke rehabilitation  for patients in the community setting. You will be working alongside a highly experienced multi-disciplinary team. There will be ample scope for individual training and development,  service development and research projects. 

This post requires excellent interpersonal and communication skills and a commitment to effective team working and skill sharing. The post holder will have experience working with adults with complex presentations, in particular Stroke. The successful candidate will demonstrate leadership qualities; and be able to evidence supporting staff/student development. There will be opportunity to contribute to service developments that are both locally and nationally driven.

You will receive regular supervision/appraisal and have access to specialist training and learning opportunities in order to support your career development in this specialist field.

As this is a community based post, access to a vehicle with appropriate business cover is strongly advantageous. 

Main duties of the job

The post holder will have the opportunity to work closely with highly skilled multidisciplinary colleagues, your named supervisor and support of the Advanced Clinical Practitioner and Therapy Management Team. 

The main responsibilities in this post include:

- To carry out an initial neurological therapy assessment to identify post stroke deficits and to clinically reason the need for further assessment and treatment.
- To identify the most appropriate form of ongoing assessment including functional assessments, seating, upper limb and need for splinting along with standardised cognitive and mood screens.
- To set patient centred goals, devise and regularly review treatment plans to achieve these.
- To work closely with the MDT to identify rehabilitation potential and the most appropriate setting for this to occur eg stroke rehab unit / own home.
- To communicate effectively with patient, family and wider organisations in order to effectively support patients recovery and co-ordinate a safe transfer of care / discharge.
- To effectively manage own caseload, prioritise and delegate to Generic Therapy assistants.
- To identify all necessary equipment to facilitate discharge and complete timely referrals to equipment supplier.
- To complete clear and concise referrals to other services and co-ordinate a timely discharge to ensure maintenance of patient flow.
- To ensure compliance with mandatory training and ensure all trust policies are followed.

  • Evidence of the advanced skills required to plan complex and rapid discharges

Knowledge

Essential criteria
  • Evidence of sound clinical knowledge and specialist skills relevant to the clinical caseload
  • Broad knowledge of current best practice in Occupational Therapy
  • Have an understanding of the legal responsibilities of the profession

Skills

  • Ability to manage own work load, determine priorities and delegate tasks
