Occupational Therapist - Specialist Palliative Care Services

Job overview

We are seeking a dynamic band 6 occupational therapist to join our expanding therapy team based at Woodlands Hospice. The candidate must have experience of working with patients with palliative diagnoses and skills specific to the role.

You will be supported by our Band 7 Clinical lead for occupational therapy and in turn will provide guidance to our Band 3 therapy assistants. You will also work closely with our physiotherapists and the wider teams. Your role will involve inpatient and outpatient work in the Hospice, complex community outreach work and input with palliative patients in the acute Trust when necessary. Experience of managing anxiety and fatigue is required as are excellent communication skills. 

Your own vehicle and a full UK driving licence will be required for this role.

Main duties of the job

To provide a high standard of occupational therapy service to LUHFT palliative care inpatients at Woodlands Hospice and ambulatory and outpatients known to the Integrated Mersey Palliative Care Team (North Hub).

Detailed job description and main responsibilities

To undertake occupational therapy assessment, clinically diagnose and implement agreed treatment plans, evaluate care delivery according to health care needs for routine and complex patients.

To ensure effective communication with patients, carers and all other health and social care professionals regarding all aspects of patient care. This will include patients who may have difficulties in understanding or communicating.

To organise and participate in teaching and advice sessions with patients, carers and other health care professionals to ensure best provision of patient care.

To represent the occupational therapy service at multidisciplinary meetings to ensure the delivery of a co-ordinated multidisciplinary service.
To carry out home assessments within the patients usual place of residence. Liaising with family, carers, and multidisciplinary team and support teams within the community. To make recommendations based on Occupational Therapy clinical reasoning, problem solving and risk assessment in order to maintain the patients functional abilities and quality of life within the community.

To liaise with statutory and voluntary agencies regarding social Care and housing, to provide equipment, recommend and arrange home adaptations to support independent living skills. To complete necessary documentation required by statutory and voluntary agencies.

Qualifications

Essential criteria
  • Degree in Occupational Therapy or equivalent
  • HCPC Registration
Desirable criteria
  • Member of Special Interest group

Experience

Essential criteria
  • Demonstrable post registration experience in a junior rotational post providing a broad experience in musculoskeletal / medical surgical conditions
  • Evidence of a contribution to clinical education of less experienced staff, students or assistants
  • Evidence of participation in audit
  • Evidence of the advanced skills required to plan complex and rapid discharges

Knowledge

Essential criteria
  • Evidence of sound clinical knowledge and specialist skills relevant to the clinical E Feb 2022 caseload
  • Broad knowledge of current best practice in Occupational Therapy
  • Have an understanding of the legal responsibilities of the profession

Skills

Essential criteria
  • Ability to manage own work load, determine priorities and delegate tasks
  • Effective written and oral communication skills
  • Ability to keep accurate legible patient notes
  • Teaching / Presentations at In – Service level
  • Able to evaluate and be critical of self
