Job overview

An exciting opportunity has arisen for a part time, permanent Band 7 Diabetes Specialist Nurse. The successful candidate will work within a nationally and internationally recognised diabetes team. This large unit is well known for its dynamic and innovative delivery of diabetes services. Working within a multi-professional team of Diabetologists/Endocrinologists, diabetes specialist dietitians, diabetes podiatrists, diabetes pharmacists and a team of diabetes specialist nurses. The role will include taking a lead on an area of the service and supporting and supervising less experienced colleagues within the team. You will be undertaking Specialist assessments and documenting management plan for multi-professional colleagues and Healthcare practitioners both within the organisations and for referral on to other teams. Main duties of the job

The role of the band 7 CNS in Diabetes includes the following: -

  • Perform specialist assessment of people with diabetes in the inpatient and outpatient environment, plan, implement and evaluate care delivery using advanced clinical knowledge and clinical skills.
  • Undertake the diabetes competency programme of education based on the national TREND Diabetes competencies.
  • Lead clinical care by managing a patient caseload providing expert assessment, planning and evaluation with the support of the wider MDT. This will include initiation and adjustments of medications including insulin therapy. Non-Medical Prescribing qualification is essential for this role.
  • Co-ordinate and manage patient reviews from the Diabetes Dashboard or other referral systems. Admit and discharge as appropriate from own caseload.
  • Contribute towards a multi-professional approach in the management of patients within the specialist area, especially in relation to ongoing care needs and discharge arrangements.
  • Support patients and their families and ensure that they receive the required information to enable them to self-manage, lead, or participate in their care delivery and to support safe discharge or referral on to other teams/GP care.

Take part in diabetes awareness programmes and national diabetes awareness activities to promote and support diabetes education and safe care across the organisation

Qualifications

Essential criteria
  • RGN with current NMC registration
  • Degree (or equivalent)
  • Post-registration qualification in diabetes
  • Teaching and assessing qualification
Desirable criteria
  • Non-medical prescribing course (V300)

Knowledge, Training & Experience

Essential criteria
  • Clinical experience with specific experience within Diabetes.
  • Sound knowledge of current developments in the NHS
  • Experience of teaching and assessing in the clinical area.
  • Experience in service development and implementation of change
Desirable criteria
  • Experience of caring for patients using insulin pumps

Skills

Essential criteria
  • Ability to effectively communicate sensitive information in a variety of settings.
  • Able to influence and overcome resistance through application of advanced communication skills
  • Able to analyse data and provide written reports
  • Intermediate IT skills in word processing and spreadsheets
  • Able to manage work autonomously
