Job overview

Do you have an interest in supporting people with chronic respiratory disease?  Do you want to be able to support people to improve their quality of life?   If so this post could be for you!

We are wanting to recruit an enthusiastic and  dedicated exercise physiologist to work as part of our innovative and supportive pulmonary rehab team in Knowsley Community Respiratory Service (KCRS).

KCRS is a large integrated community service supporting people with chronic respiratory disease across Knowsley.  The team consists of multi-professional consultant workforce,  GP's, nurses, advanced practitioners,  physiotherapists, exercise physiologists, support staff, pharmacist, respiratory physiologists, psychology, smoking cessation advisors and a large admin team.

KCRS provides diagnostic clinics and optimisation clinic, home oxygen services, admission avoidance service, virtual wards, pulmonary rehabilitation, smoking cessation and mental health support.  The team is  large and supportive, with an ethos of innovation and wanting to provide the best care possible for our patients.

Please be aware this is a community post, and the successful candidate must have a full driving licence and access to a car for work purposes due to all venues not being accessible on public transport and the need to transport equipment. 

Main duties of the job

The job will involve providing assessment for patients referred into the pulmonary rehabilitation programme.  It will include deciding suitability to exercise and providing exercise prescription and progression for patients in classes.  The post also involves supporting patients to exercise in gym environments, both in a group and on 1:1 basis as required and participating in community walks.  The will also be the opportunity to provide home visit to  support people with a home exercise programme and on a digital rehab platform.

Detailed job description and main responsibilities

To work as part of the KCRS in the delivery and co-ordination of exercise based Pulmonary Rehabilitation ensuring high quality, timely and evidence-based care.  To provide evidenced-based care to patients with a variety of underlying chronic respiratory disease, including but not limited to COPD, bronchiectasis, asthma and pulmonary fibrosis.
 
PRINCIPAL ACCOUNTABILITIES
 
CLINICAL
 
• To assess, plan, treat and review the exercise needs of patients referred for Pulmonary Rehabilitation with an underlying chronic respiratory disease.
 
• To deliver the exercise element and participate in the education program of Pulmonary Rehabilitation.
 
• To ensure that patients progress appropriately through sessions and give advice and information as necessary.
 
• To refer patients to appropriate community support programmes.
 
• To communicate effectively with members of the multi-disciplinary team to ensure holistic patient care.
 
• To provide PR in a variety of clinical settings, including; leisure centres, community venues, gyms, virtual platforms, home-based and utilizing exercise prescription platforms.
 
• To be able to work as part of a team, but also to work independently in a variety of clinical settings.

Qualifications

Essential criteria
  • Degre in exercise science or exercise physiology
Desirable criteria
  • Post graduate course in pulmonary rehabilitation

Knowledge and Experience

Essential criteria
  • Good knowledge of chronic respiratory disease
  • Experience in exercise assessment and prescription
Desirable criteria
  • Experience of working within pulmonary rehabilitation
  • Experience of working within cardiac rehabilitation
  • Experience of health promotion
  • Participation in research / audit

Skills and abilities
