This innovative consultant post works across boundaries of physical and mental health services building partnerships to best meet patient needs. The patient group have frequent presentation to healthcare services reflecting persistent physical symptoms  or long term conditions together with significant psychological distress. Hence this post is well suited to a consultant who has an passion for understanding the interaction between physical health and complex emotional and relational needs, and who values working within a biopsychosocial model of care. This post, along with a colleague psychiatrist/medical psychotherapist and two psychologists supports a small team of liaison mental health practitioners offering psychologically informed patient care for a period of up to 18 months. 

Main duties of the job

The post holder will work closely with members of the MDT providing  collaborative  assessment, formulation and ongoing management of patients referred into the service. As a senior member of the team, they will be involved in the ongoing development of the service pathways and ensuring best evidence in care. The post holder will, along with psychology and psychiatry colleagues, provide supervision to liaison practitioners in the team. They will additionally offer liaison and advice to colleagues in both physical healthcare and mental health services in the borough, and share expertise through provision of training.

Detailed job description and main responsibilities

The post holder will work alongside an existing consultant psychiatrist/medical psychotherapist (8PAs) to provide consultation for and liaison with GP practices and neighbourhood practitioners across Stockport using a collaborative care model with patients 16 years and above. This service user group have high levels of complexity who may be at risk of harm, with complex poly-pharmacy and where diagnosis/formulation is needed. 

The post holder will liaise with Primary Care clinicians and managers and Primary (e.g. IAPT) and Secondary Care Mental Health Services (such as psychiatrists, physician 
colleagues and substance misuse services) amongst others. There will also be liaison with clinicians working in primary care, GPs, practice nursing staff and community healthcare staff. 

This will involve identifying the learning needs of Primary Care and Primary Care Mental Health staff and others and developing a suitable educational and training approach to be delivered in a variety of formats and settings such as formal direct teaching, workshops and providing a space for reflection and discussion. Supervision of primary care staff, including GPs and IAPT staff is key feature. The post holder will also be expected to engage actively in education and training of the service’s own staff including liaison practitioners. The service has a GP plus post attached from a local GPs in deprivation training scheme, and the post holder will be encouraged to be a clinical supervisor.

The post holder will attend relevant project planning meetings as well as other Pennine Care and CCG/ Stockport Together events that support the delivery of the project. 

The post holder will take an active role in developing this area of work with the support of the team lead, rest of the team, IAPT services, the Psychological Medicine Service, 
Pennine Care and local commissioners. 

Clinician and patient rated outcome measures, patient rated experience measures and health care utilization data will be collected. The amount and impact of educational activity will be evaluated. This will be done with the support of existing Pennine Care and CCG structures and externally commissioned providers. The post holder is expected to take an active role in supporting and facilitating this.

Qualifications

Essential criteria
  • MRCPsych (or equivalent)
  • CCT in General Adult psychiatry (or equivalent)
  • MB BS or equivalent medical qualification.
Desirable criteria
  • Qualification or higher degree in medical education, clinical research or management.
  • Endorsement in Liaison psychiatry
  • Psychotherapy qualification at postgraduate level
  • Additional clinical qualifications.

Eligibility

Essential criteria
  • Fully registered with the GMC with a license to practice at the time of appointment.
  • Included on the GMC Specialist Register OR within six months.
  • Approved under S12 OR able to achieve with 3 months of appointment
Desirable criteria
  • In good standing with GMC with respect to warning and conditions on practice
  • Approved Clinician status OR able to achieve within 3 months of appointment

Transport

Essential criteria
  • Holds and will use valid UK driving license OR provides evidence of proposed alternative.

Clinical skills, knowldege & experience

Essential criteria
  • Excellent knowledge in liaison psychiatry including knowledge and experience of a range of persistent physical health and long-term conditions with complex psychological problems
  • Evidence of experience and knowledge of a range of psychological interventions for this patient group
  • Excellent clinical skills using bio-psycho-social perspective and wide medical knowledge
  • Excellent oral and written communication skills in English
  • Able to manage clinical complexity and uncertainty
  • Makes decisions based on evidence and experience including the contribution of others
  • Able to meet duties under MHA and MCA
Desirable criteria
  • Wide range of specialist and sub-specialist experience relevant to post within NHS or comparable service
  • Evidence of recent experience in working in a Primary Care setting
  • Evidence of recent experience of delivering group supervision
  • Evidence of experience of and enthusiasm for service development and understanding of D.O.H strategy regarding this area of clinical work and the challenges and process of implementation
