Job overview

We have a great opportunity for a Registered Mental Health Nurse to join our service! 

The Physical Health Hub is a team consisting of RGN’s, RMN’s and Healthcare Assistants with support from medical colleagues. 

The team supports Wigan community mental health teams and inpatient wards at Atherleigh Park with the following:

·       PHIT clinics

·       Lithium blood monitoring clinics

·       Advice & support in managing long term medical conditions

·       Wound care

·       Clozapine monitoring clinics and mental health support for those who take this treatment (including mental health reviews, risk assessments, care planning and completion of statutory paperwork such as MHA tribunal reports and MOJ documentation.)

We are looking for an RMN to facilitate Clozapine monitoring clinics and provide care to those who are prescribed this treatment.

It is an exciting time to join the service as we are expanding our offer to support more people with mental health needs to include Depot clinics (including MH support) and community Clozapine initiation which will be another key aspect of the role in the future to avoid the need for inpatient admissions.

There will be an expectation to support your Physical Health Hub colleagues during periods of leave, sickness etc by flexing into the other pathways within the team so this role would suit someone with an interest and/or experience in physical health.

 

 

Main duties of the job

 

 Supporting in Clozapine monitoring clinics as required

 Assist in the development and implementation of integrated care pathways, including appropriate service user referrals and models of care.

   Participate in case supervision to monitor named primary nurses’ standards of service user care and documentation.

Ensure the safe management of medicines and adhere to safe practice and principles of administration in accordance with the NMC code of professional conduct and Trust Medicines Code.

 Work in a professional and collaborative manner with the Multi- disciplinary team.

 Participate and promote positive intra-agency working.

 

 

Participate in the referral process, including the assessment of such individuals in accordance with the agreed care pathway and the completion of quality reports where appropriate.

 

  Responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care without supervision and act as Care Coordinator as required.

 Participate and assist in setting standards of service user care based on sound research findings.

 Where indicated and following completion of the relevant training the practitioner will act as a non-medical prescriber, at all times working within their sphere of clinical competence.
