Job overview

Lead and support palliative / end of life care education and training across Stockport health and social care economy.

Use a quality improvement approach to facilitate a range of hospital and community projects to develop end of life care across Stockport. 

Monitor end of life care across Stockport health and social care economy against national and local guidance and standards. 

Facilitate Stockport’s ongoing participation in the national, regional or local initiatives and audits regarding palliative / end of life care.

Encourage and share good practice and ensure lessons learned from significant events are shared and used to inform policy / guidelines.

Develop internal and external networks with professional colleagues to ensure good practice relating to palliative care is disseminated across all relevant organisations.

Foster team working across professional and organisational boundaries.

Provide leadership and direction and act in an advisory capacity in all matters pertaining palliative and end of life care education and training to the multidisciplinary specialist palliative care team and other professionals across the health and social care economy.

Undertake a clinical educator / facilitator role across the health and social care economy, supporting staff development and building confidence in palliative and end of life care.

Detailed job description and main responsibilities

Lead and support palliative / end of life care education and training across Stockport health and social care economy.

Use a quality improvement approach to facilitate a range of hospital and community projects to develop end of life care across Stockport. 

Monitor end of life care across Stockport health and social care economy against national and local guidance and standards. 

Facilitate the delivery high quality end of life care across all settings, including co-ordination of services and advanced care planning for patients and their carers/families.

Under the direction of the Stockport Palliative Care For Adults Local Provider Group, develop links at both the strategic and operational level to ensure a supportive and coherent approach to the implementation of palliative / end of life care principles that reflect national and local guidance.

Work alongside other End of Life Care QI Facilitators and the Stockport Specialist Palliative Care team in continuing to lead the implementation of palliative / end of life care principles across health and social care including local care homes.

Facilitate Stockport’s ongoing participation in the national, regional or local initiatives and audits regarding palliative / end of life care.

Encourage and share good practice and ensure lessons learned from significant events are shared and used to inform policy / guidelines.

Develop internal and external networks with professional colleagues to ensure good practice relating to palliative care is disseminated across all relevant organisations.

Foster team working across professional and organisational boundaries.

Participate in identifying training needs and competency requirements of staff relating to palliative and end of life care.

Provide leadership and direction and act in an advisory capacity in all matters pertaining palliative and end of life care education and training to the multidisciplinary specialist palliative care team and other professionals across the health and social care economy.

Act as a role model and champion for palliative and end of life care.

Undertake a clinical educator / facilitator role across the health and social care economy, supporting staff development and building confidence in palliative and end of life care.

 

Act as an educator and facilitator to give support, advice and guidance in the use and application of end of life care tools.

Responsibility for Patient Care

Undertake patient facing / bedside clinical educator / facilitator role for the purpose of staff development and support, specifically related to the general care of the dying person.

Facilitate and educate health and social care teams to plan and manage end of life care.

Promote the use of best practice, inform decision making and ensure a patient/carer centred approach to care and service delivery.

Uphold the NMC professional code of conduct at all times, to ensure the safety of clients and staff ensuring that NMC registration is up to date.

Planning and organising

Develop, co-ordinate and deliver appropriate education and training plan required to implement palliative / end of life care principles that reflect national guidance. These can include:

          Palliative care meetings (GSF) in primary care

          Care of the Dying and Individual Care Planning

          Rapid Discharge Process

          Electronic Palliative Care Coordination System (EPaCCS) 

          Advance Care Planning

          Communication Skills

          Mandatory Role Specific End of Life Care Training

Review and deliver current palliative and end of life care training packages that reflect national and local guidance.

Link in with current relevant education initiatives pertaining to palliative and end of life care and ensure consistency in messages delivered in training.

Maintain a credible and robust professional profile and keep up to date with; developments in national and local guidance pertaining to palliative and end of life care and best practice.

Will ensure own practice and delivery of training is objectively assessed on an annual basis, in line with the requirements and expectations of the Strategic Clinical Network. The assessment will contribute to the annual PDR.

Ensure any new documentation is endorsed by organisations across the health and social care economy.

Attend and participate at local, regional and national conferences.

If successful this vacancy will require an enhanced DBS check at a cost of £41.90. This cost will be deducted from your first Trust salary

Education and Qualifications

Essential criteria
  • Registered Healthcare Professional
  • Degree level education or equivalent
  • Teaching Qualification
  • Evidence of post registration study in palliative care
Desirable criteria
  • Masters or study at level 7
  • QI Qualification or Experience

Knowledge

  • Knowledge gained through experience of caring for the dying person in hospital or community
