Stockport Foundation Trust has an exciting opportunity for a Senior Nurse to join the team. The post holder will work collaboratively with other clinicians supporting the out of hospital agenda with a focus on proactive care.

The post holder will be actively involved in clinical assessment and management of patients with long term conditions and proactively be involved within the frailty agenda.

The post holder will work with the Advanced Practitioners within the neighbourhood proactive model, supporting complex patients to promote their care at home.

  • The post holder will work alongside with the Advanced Practitioner as a team.
  • The post holder will develop their expertise in first contact care and will work across traditional boundaries providing comprehensive assessment of patients with differential undiagnosed illness or exacerbation of existing conditions.
  • The post holder will work as an independent practitioner under the leadership of the Advanced Practitioner providing clinical assessment and treatment to a diverse undetermined caseload of adults requiring either crisis or ongoing care.
  • They will use their skills to provide safe, high quality, evidence-based care for patients in receipt of urgent care or whilst on neighbourhood caseload.
  • They will work in collaboration with all services all health and social care teams within the Borough referring patients onward as determined by their clinical or social needs.
  • They will work collaboratively with colleagues in acute and primary care accepting referrals for patients who can benefit from the service.

  • To take direct responsibility for Patient Care (including monitoring, diagnostics and investigations).
  • To act as an independent practitioner, under the leadership of the Advanced Nurse Practitioner, providing clinical assessment, diagnosis (where possible), and treatment (where appropriate) for patients referred to the service within the scope of professional practice.
  • To provide first contact assessment, request and interpret investigations and propose differential diagnoses with support from the clinical lead or Advanced Practitioner.
  • To triage and prioritise patients demonstrating the use of a variety of techniques to elicit the history of an illness and determine an appropriate care plan.
  • To co-ordinate intensive cares packages and provide clinical advice on care management.
  • To identify changes in a patient’s condition through clinical examination and take appropriate action to ensure the safety of the patient.
  • To undertake interventions consistent with evidence-based practice, transferring and applying knowledge and skills to meet patient needs.
  • To use the skills and knowledge to make referrals for diagnostic tests and instigate investigations to support diagnosis.
  • To evaluate the effectiveness of interventions in meeting prior agreed goals and making any necessary modifications.
  • To undertake necessary assessments and as a non-medical prescriber, prescribe within the scope of professional practice.
  • To ensure that the care provided and services delivered are in line with Stockport NHS Foundation Trust policies and procedures and the Clinical Governance Framework.
  • To maintain accurate and contemporaneous patient records in accordance with Stockport NHS Foundation Trust and national professional policies and guidelines.
  • To use clinical skills and expert knowledge to provide proactive monitoring and provide timely intervention.
  • To be aware of vulnerability within the patient/carer caseload and report potential safeguarding situations according to local policy and guidelines.
  • To work with the multidisciplinary, multi-agency team to co-ordinate the development, implementation and evaluation of care programmes for patients and their carers, therefore guarding the safety of the patient and their carers when enabling them to remain in their own home.
  • To take responsibility for developing Enhance Case Management Plans to support the reduction of unnecessary hospital admissions.
  • To educate and empower patients and carers to recognise deterioration and manage their own health conditions.

If successful this vacancy will require an enhanced DBS check at a cost of £41.90. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.
