Stockport diabetes is delighted to be successful in being selected as a site for the newly launched national Transition and Young Adult Care (TYA) Project Pilot, with all the exciting opportunities and potential this brings.  
This national project is part of the joint NHS Diabetes and CYP Transformation Programme to improve care and outcomes for children and young adults, living with diabetes, age 16 – 25 years. The role is until March 2025. Key areas of focus for this programme are improving care for those transitioning from paediatric to adult care and addressing the poorer outcomes for children and young adults with diabetes at transition age.
As a Pilot Site we have received funding to enable us to build the evidence base with regards to providing clinically effective TYA models.
The Service Specification for this TYA Pilot is a clinically led consensus approved by the National Children, Young Adults and Diabetes Oversight Group. The national vision for the pilots is that this model service specification will be implemented by all sites as a minimum standard.
There is the welcome expectation we develop innovative models of care; guided by patient led feedback and supported by different disciplines such as psychology practitioners and youth workers.
To achieve the aims of the Pilot Project we are recruiting dynamic, enthusiastic and forward-thinking multidisciplinary professionals who have experience and knowledge of working with young adults with diabetes.

Prif ddyletswyddau'r swydd

To contribute to Nutrition and Dietetic Services in Stockport by providing:

 

·       To lead and develop the dietetic intervention of the  Diabetes TYA pathway and MDT clinic

·       To support the Diabetes TYA MDT in developing, establishing and evaluating the new emerging pathway

·       Be responsible for planning, coordinating, delivering and evaluating clinical case work within the  Diabetes TYA service

·       To provide evidence based, highly specialist knowledge and skills to the Diabetes TYA service

·       To support the training, education and learning of the team and services across the system.

·       Provide a key role in partnership working with patients, carers, the MDT, and wider statutory, private and voluntary sector services.

·       To take a lead with quality improvements in the Diabetes TYA service

·       To take responsibility for staff and student training and development.

Main Duties and Responsibilities

 

  • Work as an autonomous specialist Dietitian to provide high quality service to agreed standards
  • Provide a Nutrition and Dietetic Service to the Diabetes TYA service within Stockport
  • Assist in developing and monitoring the Diabetes TYA service
  • Attend relevant meetings and liaise with medical, nursing and other health
  • Train staff, as necessary.
  • Work in conjunction with CCG medicines management department to ensure optimal use of prescribable diabetes related products, when appropriate.
  • Provide nutritional input on a weekly basis to multi-disciplinary meetings, to complete information on patient requirements
  • Promote the role of the Dietitian in primary and secondary care settings to patients and health professionals including the management of nutrition and diabetes.
  • Liaise with health care professionals to ensure consistent dietary advice is given by all team members and all aspects of care for the individual are planned, co-ordinated and integrated.
  • Maintain professional standards ensuring that clinical practice is evidence based and complies with current guidelines for good practice.
  • Assist in the updating and development of diet sheets and new teaching materials.
  • Participate in the training of dietetic students when required
  • Assist with training and updating health professionals regarding Nutrition and Dietetics
  • Collect all data necessary to meet information requirements as directed
  • Participate in quality assurance programmes and development and maintenance of service standards, policies and procedures.
  • Undertake regular, appropriate, continuing personal and professional development
  • development as agreed locally through objective setting and regular appraisal
  • Work as a committed and effective member of the Diabetes TYA and Nutrition and Dietetic team in order to       maximise the potential of the service and profession, to enable the provision of a co-ordinated service
  • Any other duties appropriate to the grade as directed by the Team Leader
  • Observe the provisions of, and adhere to, all Trust policies and procedures
  • Comply with professional guidelines and Code of Conduct
