Trosolwg o'r swydd
To participate in the provision of a high quality Community Mental Health Service for people living with a young onset or rarer dementia . This will involve the monitoring of the service users mental health and cognitive state, self-care, compliance with treatment and engagement in activities of day-to-day living as part of a multi-disciplinary team.
Attend and participate in team meetings and group supervision as well as individual supervision. Report any changes in the service user’s physical, cognitive and/or mental health to the team manager and promote living well with dementia.

- To work primarily with clients in their homes in accordance with the Trust Lone Worker Policy
- To work with qualified staff to identify clients needs and an appropriate treatment plan. This would include periods of direct observation (frequently as a lone worker), assessing for contributing factors that may cause/trigger the challenging behaviour, working autonomously having the ‘freedom to act’ to challenging situations that may occur.
- Participate in the practical aspects of care, demonstrating to care givers appropriate person centred care approaches
- Contribute to the evaluation of care plans within the patient review system, by joint discussions and attending team meetings
- Safely accompany clients to appointments and other locations according to their treatment plan .

- To work within the limitations of the post seeking help and advice when unsure and reporting problems encountered to a qualified member of staff.
- Adhere to Trust/Borough policies, procedures and guidelines
- To attend mandatory training sessions
- To participate in regular monthly supervision sessions and annual appraisals
- To escort clients/patients as deemed appropriate by senior staff
- To work primarily with clients in their homes in accordance with the Trust Lone Worker Policy

- Obtain and compile information relating to the person’s past experiences (life history). This information can assist the care giver in knowing the pre-morbid personality of the client and also used as a diversion technique in a challenging situation

- Maths and English GCSEor equivalent

- Previous experience employed in direct patient contact within mental health services OR • Previous experience of working in a care setting • Able to demonstrate a good understanding of dementia and person-centred care
