An exciting opportunity has arisen for a Registered Nurse Associate within the Physical Health & Treatment Team in Stockport
Working with patients who have been identified as having a Severe Mental Illness (SMI), you will be supporting them to access a Primary Care Physical Health Screen, reducing barriers and helping patients who cannot access services directly. You will work across Primary and Secondary Care Settings, promoting continuity of care and effective communication.

Key Responsibilities of the Post:
- Liaising with and supporting GP practices in the reviewing of the SMI register
- Working alongside our partners in Primary Care in supporting this service
- Provide support to enable patients’ to engage in meeting their health needs. This may include arranging initial appointments and attending with the service user if required, contacting them by phone and enlisting other services to support
- To support patients whose health and wellbeing is at risk, acting upon and escalating concerns appropriately.
- To provide health promotion to patients, understanding the relationship between mental health and physical health. To be able to enhance a patients overall wellbeing, and promote parity of esteem
- To undertake physical health checks, and respond to & report concerns appropriately
- To undertake venepuncture after relevant training and ongoing guidance
- To refer onto expert services in the community where a need has been identified
- Supervision of others staff members within the team, as appropriate.
- To undertake any other duties, appropriate to the grade, that may be required.

- Registered Nurse Associate with Foundation Degree
- GCSE's Grade C/Grade 4-9 or above in Math's and English Language or equivalent
Experience

- Experience of working with individuals with mental health needs
- Experience of working with individuals in distress

- Working with groups/coordinating activities for service users
Knowledge

- Understands and acts in line with NMC professional standards for practice contained within The Code
- Understanding of mental health issues and how these impact on individuals.
- Knowledge of physical health observations and their importance.

- Awareness of local services.
- An understanding of the mental health system.
- Basic first aid skills
Skills and Abilities

- Ability to communicate effectively (written, verbal and non-verbal communication) with patients/relatives and carers and all members of the multi-disciplinary team.
- Ability to support, supervise, assess and act as a role model to nursing associate students, other learners and health care support workers, as required
- Ability to organise and prioritise own delegated workload
- Ability to act calmly in emergencies and to respond in a professional manner to stressful and challenging behaviour
- Ability to plan and implement care for individuals, as appropriate
- Good IT Skills

- Willingness to engage with or have undertaken Venepuncture and Physical Health Checks training.
