HBTT is an alternative to hospital admission, supporting people to remain in their own homes whilst receiving a high level of care and treatment for their mental health.

The Team consist of experienced multi disciplinary colleagues, including Nurses, Social Workers, Psychiatrists, O/T and Psychologist all working within Recovery based model. You will have ongoing supervision with opportunities to develop via relevant training, appropriate to this role.

The role within the team will allow you to develop skills in both assessment and risk management, alongside key working service users who require full HBTT support.

Main duties of the job

An exciting opportunity has arisen for a band 6 Mental Health Practitioner to join the Home Based Treatment Team in Salford. 

We welcome practitioners from a nursing, social work or occupational therapy discipline.

This is a fantastic opportunity to join an enthusiastic and dynamic community team. Home Based Treatment Team provides an alternative to inpatient admission and support people with severe and acute mental health problems in their home whilst undergoing intensive treatment for their mental health needs.

Candidates will be expected to work as a keyworker providing assessments and inform treatment plans for those people under Home Based Treatment. You will be a good communicator and self-motivator who delivers high quality care. You will be expected to carry out in depth assessments in the community and you will be expected to contribute and formulate care plans and risk management plans.

Home based treatment team is a 24/7 service and you will be working on a rota basis, including some weekends, evenings and nights when the service requires .

  • Must have professional registration: NMC, Social Work England, HCPC

  • Post qualifying training psycho-social intervention's

  • Relevant experience in urgent care and/or community

  • Experience of managing and formulation of risk and completing assessments

Knowledge

  • Excellent risk assessment and management skills

  • Experience of suicide prevention, completing MSE
