This is a new role, which the successful candidate will be able to develop over time. Offering an exciting opportunity to work within a team which promotes holistic patient care, this post requires a skilled Advanced Practitioner Physiotherapist with an interest in working with patients with complex physical health difficulties and psychological distress. The service works with patients with persistent physical symptoms as well as people who have atypically high health care utilisation. Patients may present to the service with a wide variety of presentations and conditions. 

Applications will additionally be considered from experienced specialist physiotherapists who wish to commence advanced clinical practice training. Post holders would commence this training at band 7, and would be awarded 8a on successful completion of the training.  Further information on this route is available by contacting the service. 

The team, comprising of Consultant Psychiatrists, Psychological Therapists and Liaison Mental Health Practitioners work closely with colleagues in local Primary Care Networks as well as colleagues in the Acute trust.

The post holder will work within the Psychological Medicine Service providing biopsychosocial assessment and physiotherapy treatment for patients with long term health conditions or persistent physical symptoms comorbid with mental health or psychological distress. The post holder will work autonomously within the Trust policy and professional practice, which may involve using skills beyond the scope of professional practice. Examples of this might include requesting and interpreting the results of investigations e.g. x-rays/bloods/MRI scans to assist clinical diagnosis and in the formulation of management plans, listing for surgical procedures and referring patients to other medical and healthcare professionals. This may include being an independent prescriber, if appropriate.

Psychological Medicine has been established in Oldham since 2001 and was originally set up to work within the acute trust pathways. The service has developed and grown over time and has received national recognition from NHS England and the Kings Fund. The team currently takes referrals for Oldham patients from GPs and Primary Care Networks as well as colleagues working in community and acute healthcare. 

Pennine Care NHS Foundation Trust covers the boroughs of Bury, Rochdale, Oldham, Tameside and Stockport. Across the trust, there are a range of specialist Clinical Health Psychology and Psychological Medicine Services.

To work in clinic alongside medical consultants, mental health nurses and psychological therapists.
To perform highly specialist assessment demonstrating advanced clinical reasoning skills in the management of patients within the designated clinical speciality who will have complex and/or acute or chronic conditions comorbid with psychological or mental health distress
Determine a clinical diagnosis, develop, deliver, implement, evaluate and modify individualised treatment programmes (including managing medication, if appropriate), ensuring appropriate pathways of care and communication via liaison and referral to other agencies as required.

To act as a physiotherapy lead and a source of expertise within the psychological medicine multi-disciplinary team (MDT), education and social care.
To provide clinical leadership and undertake a team leader role if appropriate assisting the line manager in clinical governance action plans, and in strategic planning, co-ordinating, delivering and evaluating the clinical aspects within the speciality service.
To demonstrate awareness of the strategic objectives and priorities for the patients and service and act accordingly e.g. capacity, waiting time targets.
To comply with statutory requirements for physiotherapy, within the Chartered Society of Physiotherapy standards and codes of professional conduct.

Education / Qualifications

  • Degree in Physiotherapy or equivalent.
  • HPC registration.
  • Evidence of CPD in the form of a detailed personal development portfolio.
  • Specific advanced clinical courses in relevant specialist area.
  • Highly developed specialist knowledge/experience to Masters level or equivalent in a specialist clinical area.

  • Post graduate higher degree (MSc or PhD)

Experience

  • Extensive and appropriate post-registration experience.
  • Previous experience undertaking a Band 7 role working with patients with persistent pain.
  • Research and Development awareness and/or experience.

  • Contribution to service and staff development.

  • Experience of liaison with other professionals and partnership working.
  • Advanced clinical reasoning skills in the musculoskeletal field.

Knowledge

  • Highly specialist knowledge in anatomy and physiology to underpin relevant clinical skills/practice.
  • Good working knowledge of all areas of clinical governance including quality, audit and risk management.
  • Basic understanding of computers and their applications.

  • Experience of using biopsychosocial consultation skills in the management of complex patients, and of applying advanced communication skills utilising techniques such as motivational interviewing, ACT, CBT.
  • Excellent written and verbal communication skills.
  • Able to present information in a clear and logical manner.

  • Teaching, supervisory and appraisal skills and experience on a regular basis.
  • Proven organisational skills including, time management, effective delegation and prioritisation and the ability to complete a task. Self-motivated and able to lead and motivate others within a team.
  • Ability to work autonomously within specialist clinical area.
  • To comprehend and work within the Trusts policies which includes data protection, equal opportunities and Health and Safety to meet the differing needs of patients.
