Frailty Specialist Therapist

We are looking for a dynamic and enthusiastic Frailty therapist to join our friendly Frailty team as a Band 7 Specialist.  This is a fulltime post of 37.5 hours. The post holder will be working in a newly established frailty service at the Oldham Care Organisation, set up within the hospital. It is envisaged the post holder will support the same day turn around for our frail patients, by supporting them back into the community with appropriate resources.

 This role of a frailty therapist is part of a national development in delivering specialised frailty service set out by NHS England.   It requires the commitment to provide clinical support to patients, their families/carers and health professionals involved across the Frailty Pathway.  The post holder will use their clinical experience, expertise and enhanced clinical competencies to assess and treat patients. 

The role requires understanding of the particular requirements of elderly, frail patients within an acute setting and the knowledge and skills to assess patients with Frailty Syndromes.

The candidate would be expected to collaborate on team and individual goals and liaise with other departments to convey Frailty knowledge to a wider audience.

Prif ddyletswyddau'r swydd

·         Working as a Senior Frailty Clinician within an Acute Frailty Service, facilitating efficient, effective and coordinated assessment and treatment of frail older adults in the Emergency Department, on the Frailty SDEC Unit and across the Acute Frailty Pathway.

·         To participate in daily operational activities and management.

·         Be involved in the planning and development of the Frailty team, in conjunction with the clinical lead.

·         To provide professional support to qualified staff, students and support staff.

·         The post holder will require highly skilled time management and priority skills due to frequent interruptions of job tasks during the working day and balancing of clinical and operational duties, including service development.

·         Ensure clinical effectiveness of therapy intervention, incorporating evidence based practice and professional standards.

·         To work with the multidisciplinary team to provide early and pro-active Comprehensive Geriatric Assessment (CGA) to patients who meet the referral criteria. The holistic assessment will include clinical, social, functional, emotion and cognitive elements as documented within the CGA.

·         To utilise the CGA knowledge gained to initiate and facilitate early discharge plans in collaboration with community services, ensuring that patients’ needs are met and likelihood of unnecessary readmission is reduced.

Experience

  • Experience in care of the Elderly, Frailty and acute medicine
  • Extensive experience working in an acute health setting and complex discharge planning.

  • Experience of involvement in audits

Skills & Abilities

  • Effective written and verbal communication skills to a range of audiences

  • Able to work weekends in line with the 7 day working provision
  • IT skills / computer literate
  • Able to work independently as an autonomous practitioner and to work as part of a team including MDT
  • Able to maintain judgment and make decisions under pressure
