Job overview

The Community Rehabilitation Team are currently recruiting One Band 3 Rehabilitation Assistant who is passionate about providing care to residents in their own home.   One post is part time and one post is full time.

The service provides therapy for patients with a wider range of conditions who would may be struggling to access care in an out-patient setting.  The successful candidates will work with a team of Physiotherapists and Occupational Therapist as well as the wider Multi-Disciplinary Team to provide treatment and intervention as prescribed by a qualified clinician. 

The successful applicant will possess experience of working within health or social care teams, show good communication skills and be dedicated to providing high quality care to patients. 

They will also need to have a UK driving license and access to a vehicle to travel across the north of Manchester to be able to carry out home visits.

Main duties of the job

To provide rehabilitation treatment plans, to patients referred into the community rehabilitation services including physiotherapy, occupational therapy and falls management.

To work with a diverse group for patients with complex medical conditions.

To take an active part in the delivery and evaluation of care packages developed to meet patients needs under the indirect supervision of a therapist.

To work as part of a team within the service and to maintain clinical skills and competencies.

To work without direct supervision undertaking tasks delegated by a qualified health care professional.
