12 Months Secondment/Fixed Term Position 

GMMH is redesigning and expanding its forensic community mental health services moving towards an integrated model of care. As part of the redesign, we are refocusing the referral and admission pathway and are developing a Single Point of Access for AFS.  

The Triage Practitioner will have a significant and key role in the transformation and direction of the referral and assessment pathway including out of area placements (OAPs). The post holder would be expected to provide management and clinical leadership responsibility for the pathway including the Admission and Referral Panel. An ability to work within and take a whole system collaborative approach is essential as is an ability to work productively alongside different agencies and commissioners.   

This is a unique opportunity to be part of the redesign and development of a growing, highly skilled and supportive team.  

The successful candidate will report to the Service Manager for Forensic Community Services. This role will involve working alongside key colleagues and partners across Greater Manchester and the Secure Provider Collaborative. 

The post will be 12 months fixed term in the first instance. In some cases, there may be a requirement to travel long distances. Flexible working will be considered.

Person specification

Education / Qualifications

Essential criteria
  • • RMN Qualification to degree level
  • • Recognised post-graduate management qualification to Masters level.
  • • Evidence of continuing professional development
Desirable criteria
  • • Skills in presenting information to groups of pro• Experience within an inpatient/community/Social Care settingfessionals and others. • Student supervision and training skills.

Experience

Essential criteria
  • • Significant experience in secure mental health services at a senior level
  • • Experience of working in a multi-disciplinary setting
Desirable criteria
  • • Experience in managing and delivering projects across a wide range of services.

Knowledge

Essential criteria
  • • Knowledge of current national policies and legislation relevant to forensic mental health services
  • • Experience of health service management and leading on service redesign resulting in service improvements.

Skills and Abilities

Essential criteria
  • • Ability to work under pressure and to prioritise.
  • • To be able to lead, motivate and influence individuals through facilitating and fostering excellent working relationships with other colleagues across the Trust.
