Intermediate Care Physiotherapist

Job overview
The HMR Intermediate Tier Service is looking to recruit an enthusiastic and dynamic Band 6 Physiotherapist to work within a multi-disciplinary team. The teams provide patient focused assessment and treatment for people within the Intermediate Care facilities.
The Therapist will be expected to carry a case load delivering assessments and treatments both within the intermediate care units and within the patients home. .
Main duties of the job
You will be required to hold a UK driving licence and have access to your own vehicle for work purposes.
You will need to be flexible, with good communication skills and enjoy working as part of a team.
The team strives to ensure we provide an efficient and quality service to our patients with a focus on team working.
You will be self-motivated and able to work both independently and as part of a team. The team is focused to continuous improvement and service development and the successful applicant would be keen to contribute to this process.
Flexible working required for weekend, bank holiday and evenings and working across HMR Intermediate tier services will be required.
- To have responsibility for own caseload and deliver individualised assessments and interventions for patients with complex and changing needs
- To provide specialist support and education to therapists and wider teams.
- To provide line management for junior members of the therapy team

Essential criteria
- Degree or diploma in Physiotherapy

- Post graduate training in specialist areas
- Clinical Educators accreditation
Knowledge, Training & Experience
Essential criteria
- Involvement in and responsibility for service development activity
- Enable patient choice and participation and support others to do so
- Portray a professional image
- Ability to use advanced clinical reasoning, making professional judgements in complex and challenging situations
- Experience of liaison with other professionals and partnership working. Post graduate experience in acute medical wards and complex discharge planning
- Specialist knowledge of anatomy and physiology to underpin relevant clinical skills and clinical reasoning

- Excellent written and verbal communication skills
- Excellent organisational skills including time management, prioritising, and the ability to complete a task

- Presentation skills
