We are looking for an experienced registered Learning Disability Nurse with the above passion and skill set to be a care coordinator in the adult learning disability service. Come and join us and experience the difference that we can make together.

The role will involve leading and contributing to the effective provision of services to people with a learning disability and additional needs e.g. autism.

Using your broad range of communication skills and an evidence-based approach to care, you will work directly with individuals that use the service, their families and the people who support them in the assessment and planning processes including supporting people with complex and changing health care needs.

Together with the Community Matron you will develop the nursing service within the designated Community Learning Disability Service to manage the waiting list and referrals through allocation of work, supervision and support.

In collaboration with the senior nursing and  health managers, develop, lead and manage the nursing resource within the community learning disability team.  To receive and respond to the needs of referred learning disabled adults to develop positive lives in their local community. This involves allocation of clinical work to nursing colleagues, to ensure appropriate allocation of nursing resources and the effective deployment and development of nursing skills. To ensure maintain clinical excellence in the Learning Disability nursing field, and lead of pathways of care. In the absence of the Professional Lead the post holder may cover identified responsibilities.
