Job overview

The Home Treatment Team offers intensive home treatment to people presenting in mental health crisis as an alternative to inpatient admission or to facilitate early discharge from the acute mental health inpatient wards. 
The role will involve direct patient contact either at the service user’s home or at the hospital site, implementing an agreed care plan and supporting the team by leading on the completion of physical health screening/monitoring. Elements of the care planning will include monitoring mental state, concordance with treatment and engagement in activities of daily living. 
The post holder will also introduce service users to voluntary / 3rd sector 
organisations (Mental Health Days Centre, MIND etc.) as a means of supporting recovery and also assist services users with any unmet social needs they may have by assisting them to attend the appropriate services (Housing; Welfare Rights etc.). 
Work is supervised from a distance as the post holder is expected to work with a level of autonomy which does not require ongoing direct supervision, however, a qualified nurse / social worker will be available for reference and support as required. 

Main duties of the job

Assist the qualified mental health practitioners in assessing, planning, 
implementing and evaluating individual care plans.
Develop good working relationships with 3rd sector and voluntary agencies.
Develop a good knowledge base of local resources available to service users.
Support service users to engage with appropriate services and resources. 
Promote independent living  within the community through help, advice, and signposting.  
Provide information and support to minimise social isolation. 
Accompany service users to appointments where appropriate. 
Maintain accurate legible patient records. 
Provide information and support to family/friends of service users.
Accompany qualified practitioners on joint visits to service users where 
appropriate.
Complete in-reach work with services users referred to the HTT and who are admitted to the inpatient mental health unit.
Attend team meetings.
Actively partake in management supervision to develop and maintain safe practise.
Actively partake in IPDRs and plan for areas of personal development.

Detailed job description and main responsibilities

• To assist the qualified mental health practitioners in assessing, planning,

• To take the lead with physical health screening/monitoring.
• To develop good working relationships with 3rd sector and voluntary agencies 
and to develop a good knowledge base of local resources and opportunities 
available to service users; keeping an up to date file of local resources which 
can be shared with the rest of the team.

services and resources such as benefits, employment, housing and substance 
misuse services. 
• Promote independent living of service users within the community through help, 
advice, practical and emotional support and provide information and support 
with health promotion.
• To provide information and support to enable service users to engage in 
meeting socialisation, education and employment/vocational needs. To include 
initial contact and attending with the service user as required. 
• To accompany service users to appointments such as outpatient clinics where 
appropriate. 
• Maintain accurate legible patient records in accordance with Pennine Care 
NHS Foundation Trust Policies and Procedures. 
• Provide information and support to family and friends of service users.
• To accompany qualified practitioners on joint visits to service users where 
appropriate.

Person specification

Education/Qualifications

Essential criteria
  • Good general education, numerate and literate.
  • Commitment to undertake Continuing Personal Development.
  • NVQ level 3 or equivalent experience.

Experience

Essential criteria
  • Experience as a worker, service user or carer.
Desirable criteria
  • Community experience.
  • Experience of working within acute psychiatry and with service users experiencing mental health crisis.

Knowledge

Essential criteria
  • Knowledge of physical health and physical health observation parameters.
  • Knowledge of diversity and Anti-discriminatory practice/equal opportunities.
Desirable criteria
  • An understanding of mental health and the mental health system.
  • Knowledge of benefits/employment systems.
  • Awareness of local services.

Skills and Abilities

Essential criteria
  • Ability to form positive therapeutic relationships with service users and carers.
  • Ability to provide practical support with daily living activities.
  • Ability to create innovative solutions to help empower service users.
  • Ability to manage self effectively under pressure.
Desirable criteria
  • Ability to work sensitively with service users who misuse alcohol/illicit substances.
  • Experience with delivering relaxation/mindfulness.
  • Ability to work sensitively with people with mental health crisis
