A&E Patient Flow Tracker

Northern Care Alliance NHS Foundation Trust are looking to appoint an A&E Patient Flow Tracker to work in the busy A&E Department at Fairfield General Hospital.

This role is to ensure tracking and flow support is provided on a daily basis to the Emergency Department teams to prevent unnecessary delays and improve the patient journey.  The post requires flexibility to work days, evenings, nights, weekends & bank holidays.

Track all patients through their journey within the Department.

In collaboration with the Nurse in Charge (shift Co-ordinator) be proactive in identifying all patients who may breach the quality indicators including four hour breaches and twelve hour trolley waits.

In order to achieve the 4 hour target cascade/expedite potential delays to the NIC/Co-ordinator.

Liaise with the bed managers and ward staff regarding bed requirements.

Contact specialties, under the direction of the co-ordinator, to facilitate timely speciality response - escalating any issues to the Co-ordinator and/or Nurse in Charge.

Attend Nursing and Tracker handovers.

Liaise with internal and external supporting specialties, departments and services to facilitate movement of patients within the 4 hours.

Liaise with Estates/Path Lab if POD broken, Liaise with IM&T as required, using the appropriate escalation procedure.

Provide reports where requested.

Scan patient notes and other relevant documentation prior to transfer/admission using all necessary systems (Symphony/EMIS Tracker etc.).

A key element of this role is to ensure accurate and timely data entry onto Symphony, EMIS Tracker and other systems which reflects the patient journey in real time to support the Emergency Department and Primary Care Streaming in achieving required targets.

The Patient Flow Tracker will escalate potential delays to the relevant team in a timely manner. 

The ED Patient Flow Tracker will work closely with all multi-disciplinary teams (clinicians, nursing staff, therapy staff, Managers, Bed Managers, Wards and Departments, Rapid Response, Local Authorities, PTS, Navigators and Primary Care Services etc.) ensuring the quality indicators are met.
