We are seeking to recruit an experienced and motivated individual to the role of Advanced Practitioner to work within the Intermediate Care Tier, we would welcome a clinician with a special interest in the Frail Older person.

This is an exciting opportunity to join a multidisciplinary team encompassing both health and social care alongside links with community, primary and secondary care. We are seeking an individual with excellent clinical skills, a good working knowledge of acute conditions as well as Frailty and Elderly Care expertise. The successful individual is required to have strong communication and leadership skills and will be a dynamic and forward-thinking Clinician with a progressive approach to enhancing patient care and safety.

We would consider individuals who have completed the first year in MSc in Clinical Skills, preferably a V300 prescriber.

An autonomous practitioner, the post holder will take an active role in the advanced assessment, management and treatment of patients over 18 years of age. The practitioner will provide comprehensive healthcare sensitive to the needs of the population, including advanced clinical assessments, take history, diagnose, plan treatment, prescribe and evaluate. Leading the multi - disciplinary team across the primary, secondary and social care interfaces in Bury, you will be responsible for coordinating and meeting the health care needs of people at risk of hospitalisation or requiring a more intensive level of care in their home environment.

The post holder will provide specialised input as an independent and interdependent practitioner developing and leading relationships with members of the multi-professional team allowing for consultation and collaboration in a variety of health care settings. This will be to prevent unnecessary acute hospital admissions and keep patients in their own homes for longer and prevent premature admissions into long term care facilities.

This vacancy is open to those who have a current NMC PIN to practice as an Registered Nurse or Midwife in the UK, or those who are currently enrolled on a course which means they expect to receive their full NMC PIN within the next 3 months. Please note an NMC PRN PIN is not a full NMC PIN and would not fulfil the essential criteria for this role.

Qualifications and Experience

  • Registered Nurse or HCPC
  • MSC in Advanced Clinical Practice
  • Non Medical Prescriber
  • Masters Degree in clinical subject
  • The role requires ongoing training and compulsory CPD in order to maintain competence in the knowledge of medicines management, therapeutics, clinical governance, primary care and communication skills.
  • To demonstrate an awareness and understanding of the factors that contribute to good health and the importance of promoting these in line with the Trust’s public health aims especially with regards to the cessation of smoking.
