We are looking to recruit a motivated and enthusiastic Community Support Worker to join our new Community Rehabilitation Team. The successful candidate will have experience of working with people with severe mental illness across a range of services.
The team has a strong emphasis on the Recovery Model, working with service users to maximise their independence and autonomy to improve their quality of life. There is a team approach to ensure service users receive the most appropriate clinical interventions and they will benefit from a truly multidisciplinary team.
You will work with a team of experienced practitioners with a strong focus on psychological formulation driven, evidence based interventions.
You will receive regular supervision and your personal development priorities will be shared and supported through our appraisal process.
The team is a trust wide service covering all five boroughs across the Pennine Care footprint so a driver's license and access to a vehicle is essential for this role.
We are currently based at Trust HQ in Ashton Under Lyne although this may be subject to change. Our core hours are Monday to Friday 9am-5pm.

To work as part of the multidisciplinary team and provide support on both an individual and group basis to individuals experiencing complex psychosis.
To support service users to achieve their full potential through accessing resources within their wider communities and equipping them with the skills and confidence to enable them to manage their lives independently.
The role will involve delivery of therapeutic recovery focussed and community activities, and individualised treatment plans under the guidance of registered practitioners within the team.

To provide support to an allocated group of service users, promote their recovery and help to maintain them in their community environment.
To assist registered practitioners in assessment, planning, implementing and evaluating individual care plans.
To ensure that service users engage effectively with the agreed goal plan and access appropriate services.
Positively promote independent living of service users within the community.
Report regularly to social inclusion workers and keyworkers.
To promote independence and choice.
Provide support and interventions to service users in their own homes and the wider community based upon their assessed needs.
Support service users to become involved in social activities in line with their assessed needs and planned care.
Support service users to establish and maintain effective interpersonal relationships.
Provide regular and practical support for service users and their carers in managing dignity and independence.

- Level 2 qualification plus training and/or experience to Level 3
- Good standard of general education including good numeracy and literacy skills.
- Evidence of recent further development / training.
- Experience of working with individuals with mental health need
- Understanding of mental health issues and how these impact on individuals.
- Good written and verbal communication skills
- Ability to form positive therapeutic relationships with service users and carers.
- A current driving licence with business insurance and access to a car during the working day is essential
