Job overview

This post is to support the delivery of the integrated respiratory service and respiratory virtual wards within Hillingdon borough.  The appointee will join the existing multi-disciplinary respiratory team, and work in conjunction with their team members in coordinating and developing the service.  The post holder will work collaboratively with healthcare professionals and stakeholders involved in the provision of integrated care and virtual wards, to ensure joined up care for patients across acute, community and primary and secondary care settings.

The Hillingdon Hospitals NHS Foundation Trust is the only acute Hospital in the London Borough of Hillingdon and offers a wide range of services including accident and emergency, inpatient care, day surgery, outpatient clinics and maternity services. The Trust’s services at Mount Vernon Hospital include routine day surgery, delivered at a modern treatment centre, a minor injuries unit and outpatient clinics.

Detailed job description and main responsibilities

  • To be a lead clinician in the area of integrated respiratory care and accept clinical responsibility for a designated caseload of patients appropriate to the post.
  • To be a lead clinician for the Acute Respiratory Virtual Wards and the development of new pathways.
  • To be professionally and legally responsible and accountable for all aspects of your own work, including the management of patients in your care.
  • To ensure a high standard of clinical care for the patients under your management and those of more junior staff, using resources effectively to manage competing priorities and demands across the team and wider service.
  • To be responsible for providing comprehensive specialist respiratory assessment of patients, including those with a complex presentation, using investigative and analytical skills and with a holistic approach.
  • To interpret and analyse clinical and non-clinical facts to form diagnoses and prognoses in a range of complex presentations and to recommend the best course of intervention.
  • Formulate and deliver an individual respiratory/physiotherapy management and treatment programme based on a sound knowledge of evidence based practice and treatment options using advanced clinical reasoning skills and knowledge with documented clear clinical reasoning process
  • To monitor, evaluate, and modify treatments and interventions in order to measure progress and ensure effectiveness of treatment
  • To provide spontaneous and planned advice, teaching and instruction to relatives, carers and other professionals, based on theoretical academic knowledge and relevant practical experience.
  • To be responsible for patient care plans and ensure documentation is completed according to professional and local standards, and to ensure more junior staff do the same.
  • To understand how interventions may change in response to social, cultural or psychological needs.
  • To have knowledge and ensure appropriate use of outcome measures (both standardised and non-standardised) to measure the efficacy and effectiveness of interventions.
  • To assess and manage clinical risk within own patient caseload, and support the team in managing and minimising risks related to patient discharges.
  • To delegate responsibility to more junior staff and to take delegated responsibility from more senior staff as appropriate.

Essential criteria
  • Diploma or degree in Physiotherapy
  • Current HCPC registration
  • Teaching Certificate
  • Extensive knowledge and experience in the same or related field
  • Experience of caring for respiratory patients
  • Demonstrate knowledge of BTS/NICE/GOLD guidelines
  • Demonstrate knowledge of pulmonary rehabilitation and oxygen guidelines
  • Demonstrate high standard of clinical care
  • Sound respiratory assessment and clinical skills
Desirable criteria
  • Previous experience of integrated care, pulmonary rehabilitation and domiciliary oxygen
  • Performing arterial/capillary blood gasses
  • MSc or evidence of equivalent experience/post graduate courses
  • ARTP Certification
  • Non Medical prescribing
