Childrens' Continuing Care Nurse Assessor & Transition Coordinator

NHS North East London Integrated Care Board (ICB) was formed on 1 July 2022.
We are an exciting new organisation putting the needs of communities at the heart of our commissioning arrangements.
We are working with local people and partners across health, social care and the voluntary sector in innovative ways to improve health and wellbeing for all our communities. Our key priority in the next year will be to develop closer integrated and collaborative ways of working with all our partners to implement our Out of Hospital Strategy.

The role is responsible for delivery of:
- Continuing Care Assessments;
- Annual/Needs Reviews;
- Presenting Cases to Panel;
- Brokering Care Packages;
- Quality Assurance of Care Packages;
- Support for Young People and their Families for the Transition to Adulthood.
- Other activities related to Childrens’ Continuing Care in the borough
The role is supported by a wider integrated team, including:
- Bart’s Health- Continuing Care Service and Acute Paediatric Services,
- London Borough of Tower Hamlets Children with Disabilities Service,
- London Borough of Tower Hamlets Special Educational Needs Service,
- East London Foundation Trust Community Learning Disability Service and CAMHS,
- Local Schools, including Special Schools
- Commissioning Colleagues from NHS NEL and LBTH,
- Others
The role is responsible for ensuring that continuing care processes are delivered in line with the Children and Young People’s Continuing Care National Framework (2016). The post holder will also coordinate with health, social care and education colleagues in planning integrated packages of care to ensure they meet patient/family needs effectively. The role will ensure that public funds are used effectively and sustainably in the brokerage of care packages. The post holder will link with commissioning colleagues in the transformation of local services linked to continuing care.

Required for the role :
- Registered Nurse with a current NMC registration or equivalent
- Significant experience of working with complex health needs
- Ability to learn Continuing Care, Social Care and Education guidance and processes
- Strong knowledge base around safeguarding children
- Strong joint working and collaborative working skills
- Evidence of continued professional development
- Evidence of negotiation skills to support brokering packages
- Effective presentation skills to support presenting cases at panel
- Competent in IT including good typing skills. Able to work with a database, Excel, Microsoft
This is an opportunity for both personal and professional development through working jointly with partners to plan care for children and young people with complex needs.
The successful candidate for this post will need to be self-motivated and able to work under their own initiative, work autonomously as well as within a team and have excellent time management skills. People management skills and the ability to deal with difficult and challenging situations is also essential.
