To work as an autonomous advance practitioner providing diabetes care in both inpatient and outpatients care setting (training will be provided). To work as an autonomous advanced practitioner and effectively deliver an expert service within the specialist area both inpatient and outpatients. To manage a caseload of patients/clients. To ensure that patients and their families are fully informed and supported by providing clinical and social support. To lead in the development of evidence based clinical practice in line with current research and guidelines. To establish clear lines of communication within the speciality team to ensure cohesive multidisciplinary management of patients. Perform the role of educator to student dieticians, medics, nursing staff, dietetic assistant and other AHP’s as required.

Due to resignation , An exciting opportunity has arisen within the diabetes team. We are looking for an enthusiastic Band 7 Diabetes Specialist Nurse to join our friendly team.
The Diabetes Centre at St George’s University Hospitals is at the forefront of advances in diabetes care and provides a number of specialist services. The team includes the Diabetes Specialist Nurses (DSNs), Diabetes Dieticians, Diabetes Consultants, and our Diabetes foot team. We provide care and support for our in-patient, outpatient, and we are a very established DAFNE and Insulin Pump Centre. We have a joint Obstetric / Diabetes service, Continuous Glucose Monitoring service, and a busy renal and diabetes foot clinic.
The successful candidate will be required to:
- To work as an autonomous advance practitioner providing diabetes care in both inpatient and outpatients care setting (training will be provided)
