Job overview

Please note that this job is ward based at Teddington Memorial Hospital.

We are looking for an enthusiastic and clinically experienced pharmacist to join our team.  This Community Services based role will lead and deliver specialist clinical pharmacy services to the two inpatient rehabilitation wards at Teddington Memorial Hospital. This will involve working with the Pharmacy Team at Hounslow and Richmond Community Healthcare Trust (HRCH) and the contracted community provider, Your Healthcare (YH).

HRCH and Kingston Hospital NHS Foundation Trust (KHFT) are going through an exciting time of joint working to become one organisation.   As such, the post holder will be dynamically involved in defining new processes, supporting change and working across HRCH, KHFT and YH to deliver joined up care.

Main duties of the job

You will be responsible for the delivery and development of pharmacy services working as a member of the Multidisciplinary Team, attending weekly ward rounds. You will also work closely with Clinical Nurse Leads and Quality and Risk Leads to provide and develop safe and effective systems in Medicines Management across the organisations.

Support will be provided by the Pharmacy Team at Kingston Hospital NHS Foundation Trust with the department providing additional clinical support, medicines information, dispensary and stores services.

You will need to have a Clinical Diploma or equivalent, possess excellent communication skills and have a clear understanding of medicines optimisation in elderly patients.

Detailed job description and main responsibilities

Responsible for the delivery, development and evaluation of clinical pharmacy services to the Community Teams within Hounslow and Richmond Community Healthcare Trust (HRCH) and in line with relevant Service Level Agreements agreed with the contracted community provider Your Healthcare CIC (YH).

To lead and deliver specialist clinical pharmacy services to the Community inpatient rehabilitation wards and services as part of an integrated pharmacy team.

Provide professional pharmaceutical support to multidisciplinary community teams across Kingston and Richmond localities.

Develop and deliver medicines-related training to Community nursing staff & carers, non-medical prescribers and allied healthcare professionals.

To establish good working relations with partner organisations including YH, HRCH and SWL ICB.

Ward based at Teddington Memorial Hospital, working in close association with GPs, Clinical Nurse Leads, Quality and Risk Leads, the Medicines Management Lead and Controlled Drug Accountable Officer to provide and develop safe and effective systems and services across the organisation.

Qualifications

Essential criteria
  • Registration with the General Pharmaceutical Council as a Pharmacist
  • Post Graduate Clinical Diploma
  • Taught Master’s Degree in Pharmacy
Desirable criteria
  • Independent prescriber qualification

Experience and Skills

Essential criteria
  • Comprehensive post-registration training and experience in hospital pharmacy - this must include a solid grounding in all aspects of acute hospital pharmacy practice (medicines information, ward services and dispensing)
  • Experience of providing clinical pharmacist services to elderly care wards or medical wards as a Band 7 Pharmacist
  • A clear understanding of medicines optimisation in elderly care patients
  • Multidisciplinary teamwork
