Job overview

Are you a nurse or AHP looking for a change and to be involved in leading a service improvement project?  We have an exciting new role for a Clinical Assessor to lead the patient eligibility assessment process for patients seeking access to NHS non-emergency patient transport services.  

Working alongside the patient transport service administrative team, you will lead on the clinical triage and assessment of our patient's eligibility to receive this service.

There will be the opportunity to develop the service as part of a service improvement initiative, engage with clinical teams across the hospital, and complete a service evaluation.

We are looking for someone with excellent clinical triage and assessment skills, able to review patients from clinical notes and on the telephone.  You will have excellent communication and interpersonal skills along with organisational, leadership and IT skills.  You will have an interest in service improvements, the role good quality data plays in this, and be able to demonstrate the ability to work with multidisciplinary teams across the hospital to drive change.

The role is 9am-5pm Monday to Friday and a work from home day is available once established in the role.  The role is fixed term for 12 months.

Main duties of the job

  • The Clinical Assessor is an active member of a team that provides a high quality, evidence-based and clinical led patient transport eligibility assessment process for patients at The Royal Marsden.
  • As a skilled clinical practitioner, the post holder is a source of knowledge, subject expert and specialist in patient transport eligibility and expertise for patients, relatives, and colleagues of all disciplines. 
  • Working with Trust clinical teams they will lead and manage the eligibility assessment process to ensure patients most in need of the service are able to receive it. 
  • They will coordinate a service evaluation of the assessment process and be able to instigate service improvements.

  • To manage all first line clinical appeals and coordinate clinical documentation and evidence to present for case review by Assistant Clinical Business Unit Manager at Trust 2nd line appeal process for patients seeking review of their eligibility assessment. 
  • Work with clinical teams and Consultants across the Trust in the provision and receipt of complex and sensitive clinical information about the assessment of a patient’s clinical management to provide patients with a clear clinically evidenced outcome to their patient transport eligibility.
  • To refer and signpost patients to financial and welfare support.  To signpost patients in accessing wider external services that provide support to patients.
  • Maintaining contemporaneous and high-quality patient documentation as part of the assessment process.
  • Will be able need to communicate complex information on the clinical outcome of a patient’s assessment which may involve challenging conversations with patients and their relatives.      

 

Person specification

Education/Qualifications

Essential criteria
  • Diploma/Degree level study in a relevant discipline: nursing, occupational therapy, physiotherapy
  • Relevant professional registration eg NMC
Desirable criteria
  • Post graduate clinical qualification or specialist training

Experience

Essential criteria
  • Sufficient post registration nursing experience in oncology or speciality to be confident and competent in using the required skills to do the job
  • Experienced in patient assessment in a triage or pre-assessment role
  • Evidence of experience of dealing with oncology patients and their families / carers.
  • Evidence of experience in clinical supervision and appraisal.
  • Evidence of experience of principles and practice in audit, outcomes measurement and clinical governance
  • Project management or service evaluation experience

  • Experience in Clinical Management

  • Evidence of effective interpersonal skills with particular ability to liaise with multidisciplinary team members
  • Evidence of working cooperatively in a team

  • Evidence of commitment to customer care initiatives
  • Knowledge and understanding of quality issues
  • Computer Literacy (e.g. use of Microsoft Word, Excel and Outlook)
  • Experience of database management and using patient information systems
  • Evidence of collating, reviewing and presenting data in a variety of formats and presenting to different audiences
  • Evidence of evaluating processes and identify solutions to problems
  • Ability to coordinate as department

  • Experience in oncology or area of specialty
