Meadow House Hospice is excited to announce the expansion of our palliative care service and we are looking for two dedicated Band 6 Community Nurses in Specialist Palliative Care to join our team. These are new roles for the hospice, so it offers the opportunity for staff to be at the frontline of developing and leading on the way we deliver care to our patients, their families and how we offer support to our partners in caring.

We are looking for someone who is passionate about delivering exceptional palliative care. They must be skilled in collaborative working with a variety of healthcare professionals and will be supported by senior staff in assessing and managing a caseload of patients. They must be committed to improving patient and carer experience and supporting our service expansion and development.

Delivering an evidence-based Palliative Care Service to the adults of Ealing and Hounslow.

 

Working collaboratively with the Primary Care Team, acute sector, Social Services, and other agencies, to establish close working relationship that is essential in ensuring that care is coordinated and responsive to individual need.

 

Supporting the specialist palliative care team in managing patients and their carers ongoing needs.

 

Monitoring and assessing the holistic well-being of palliative care patients and their families under supervision.
