Job overview
We are seeking an enthusiastic and motivated nurse to join the Mid Beds Locality to work within the Community Matron Team, providing proactive case management for people with long term conditions and minor illness assessments for patients that meet the service criteria. The post holder will support the 2-hour response and Virtual Ward requirement for any person requiring a physical review or assessment within the community. You will be a strong leader, focused on achieving real improvements in outcomes for patients and will be confident in your ability to support, coach and develop individuals and teams to deliver consistently high quality services. You will be innovative, forward thinking, self-driven with excellent communication and interpersonal skills. You will be an experienced Registered Nurse with top to toe assessment and independent prescribing skills with a background in minor illness or chronic disease management. You will take pride in your work and be highly motivated and dynamic, have a flexible approach and be able to embrace change.
Main duties of the job
The post holder will be responsible for a caseload of people living with long term conditions, provide proactive assessment and prescribing for individuals in the community to reduce reliance on secondary care services and increase provision of care in a primary, community or home environment in order to prevent unnecessary hospital admission and conveyance.You will make contact with aligned care homes to see and treat patients who require review and assessment.

Detailed job description and main responsibilities
Being a collaborative worker and the ability to network and build relationships with a diverse group of stakeholders is essential. You must have a driving licence and have access to a vehicle in order to carry out your duties. You will be required to work a shift pattern covering 7 days a week from 08:00-20:00

- Registered General Nurse
- Non Medical Prescriber
- Evidence of significant post registration nursing experience including community setting
- Experience of management of long term health conditions
Desirable criteria
- Understanding of safeguarding and essentials of managing complex cases
- Experience of health promotion
- Holds qualification to teach and mentor others
- Be able to provide strong clinical leadership and change management skills
- Minor Illness experience

- Current driving licence and the ability to travel across Bedfordshire to meet the needs of the service.
- Understanding of the interface between primary and secondary care
- Experience of Caseload management
Desirable criteria
- literate in IT / Computer Skills
- The ability to collect and collate data
- Undertake the moving and positioning of patients and equipment adhering to the manual handling policy when in clinical practice.
