Job overview
The post holder will be responsible for line managing a Continuing Healthcare (CHC) clinical team.
Supporting the CHC Review and Assessments Manager and taking responsibility for the management of the day to day work assigned to the clinical service line. Ensuring that the clinical team, when carrying out their duties, work in accordance with the latest statutory requirements, in particular The National Framework for NHS Continuing Healthcare and Funded Nursing Care and that the practice is in line with the Good Practice Guidance revised 2012 and within the Code of Professional conduct NMC April 2015.
Main duties of the job
- Manage the Team’s, including developing and managing the team’s business plan, progress and reporting risk and issue management.
- Liaise with appropriate functions within the ICB
- Case management across all age Continuing Healthcare
- Take a lead in the coordination of training & development and recruitment activity across the Team.
- The above is only an outline of the tasks, responsibilities and outcomes required of the role. The job holder will carry out any other duties as may reasonably be required by their line manager.

Education / Qualifications
Essential criteria
- Educated to degree level in relevant subject or equivalent level qualification or significant experience of working at a similar level in specialist area
- Qualification in nursing/social work or other relevant qualification.
Experience
Essential criteria
- Evidence of working knowledge of National Frameworks for Continuing Healthcare (CHC Adults)
- Experience of case management in CHC
- Experience of leading a team of assessors
- Experience in investigating complaints
- Evidence of establishing and maintaining effective working relationships with NHS, Local Authority and Independent Stakeholders.
- Significant experience of successfully operating in a politically sensitive environment
- Member of relevant professional body
- Experience of managing risks and reporting
- Experience of drafting briefing papers and correspondence
- Experience of monitoring budgets and business planning processes
- Experience in a healthcare environment
Desirable criteria
- Evidence of continued professional development
- Experience of co-ordinating projects in complex and challenging environments
Knowledge
