An exciting opportunity has arisen for a Band 7 Macmillan Personalised Stratified Follow Up (PSFU) CNS to cover 12 months maternity leave,starting in October 2024, at Barking, Havering and Redbridge University NHS Trust. The position is full time, covering Monday to Friday and is an integral part of the Personalised Cancer care Team (PCC). The PCC team are a multi-disciplinary team providing personalised care and support for patients living with a cancer diagnosis from diagnosis and all through the patient’s pathway.

You will be responsible for leading on the continuing implementation of PSFU for Breast, Colorectal and Prostate cancer patients. The current service is funded for 2 years by Macmillan and this 12-month post will cover the end of the first year and start of the second year of funding and implementation. This role involves supporting patients to enable them to self manage and gain confidence following their cancer treatment. Patients results are monitored and tracked using a Remote Monitoring System. We also manage a triage line that allows patients to contact us with any problems following their treatment and we can signpost accordingly.

The post holder will be responsible for all aspects of the Remote Monitoring Pathway for patients.  They will act as the expert and superuser for the Remote Monitoring System (RMS) module of the Somerset Cancer Registry.  This will include writing and developing protocols for remote monitoring and supported self management in line with regional and national best practice, enrolment of patients, monitoring of patients on a stratified follow up pathway, escalation of concerns where necessary.

The post holder will be the lead in promoting the uptake of RMS and educating others in using the system to meet expected KPI’s and targets for RMS and Personal Stratified Follow Up. They will cultivate strong links with appropriate MDTS’s to identify suitable patients,

We also provide outpatient services at Brentwood Community Hospital, Barking Hospital, Loxford Polyclinic, and Harold Wood Polyclinic. We have two of the busiest emergency departments in London.

As we recover from Covid-19, we’re proud to be leading the way nationally in reducing the time our patients wait to get the treatment they need. The pandemic provided the drive for our teams to innovate and change.

We know we have more work to do to improve waiting times for urgent and emergency care, and our performance against the four-hour emergency access standard remains challenged, in comparison to many other London trusts.

We are particularly proud of our regional Neurosciences Centre; Radiotherapy Centre; Hyper Acute Stroke Unit; and dedicated breast care service at King George Hospital. We’re also pleased to be part of the NEL Cancer Alliance.
